Physiotherapy for ACL injurie solved by Internal Brace surgical method

Abstract:

Cílem této práce je shrnutí dostupných teoretických poznatků o fyzioterapii u poranění ACL řešené chirurgickou metodou Internal Brace a zpracování kazuistiky pacienta s touto diagnózou. Jde o práci výzkumného charakteru typu případové studie (kazuistiky) u pacienta s kompletní rupturou ACL, řešené pomocí chirurgické metody Internal Brace a bylo zde použito 3 hlavních metodologických nástrojů. Odebrání kompletní anamnézy formou rozhovoru, pozorování a testování. Jako součást vyšetření bylo provedeno měření izometrické svalové síly na siloměrném křesle a dále stabilita stoje na silových deskách Kistler v laboratoři pro studium pohybu. Na základě vstupního kineziologického rozboru, byl navržen krátkodobý rehabilitační plán, který byl realizován během 3 měsíců v rámci 15 individuálních terapií. Během intervence bylo po 4 týdnech terapie provedeno kontrolní měření. Po 3 měsících terapie byl proveden výstupní kineziologický rozbor a na jeho základě navržen dlouhodobý rehabilitační plán. Pomocí komplexní fyzioterapie se u pacienta podařilo odstranit otok, uvolnit měkké tkáně a zkrácené struktury, obnovit rozsah pohybu v levém kolenním kloubu, zvýšit svalovou sílu LDK, zlepšit osové postavení a spirální sešroubování LDK reedukovat stereotyp chůze, zlepšit statickou stabilitu obou DKK, zlepšit posturální stabilitu a upravit postavení pánve. Zlepšení svalové síly a statické stability je podloženo výsledky z funkčního laboratorního měření. Velice si vážím této spolupráce, díky které jsem měl možnost pracovat se zajímavou a inovativní metodou, kterou může být řešena ruptura ACL a která mě obohatila do další praxe. Pacient si spolupráci pochvaloval, je s výsledky společného snažení spokojený a mohl se aktivně a bez omezení vrátit do života jako před zraněním.

Abstract:

The aim of this work is to summarize available theoretical knowledge about physiotherapy in ACL injuries solved by the Internal Brace surgical method and the treatment of a patient with this diagnosis. This work is of a research character using a case study (case report) of a patient with a complete rupture of the ACL, which was solved using the Internal Brace surgical method where three main methodological tools were used. Taking the complete medical history through interview, observation and testing. As part of the examination, the measurement of isometric muscle strength on the load cell was performed, as well as the stability of the standing position using the Kistler force plates in the laboratory for a motion study. Based on the initial kinesiological analysis, a shortterm rehabilitation plan was designed and implemented during 3 months with 15 individual therapies. During the intervention, control measurements were made after 4 weeks of therapy. After 3 months of therapy, an outcome kinesiological analysis was performed and a long-term rehabilitation plan proposed. Using comprehensive physiotherapy, the patient managed to remove swelling, loosen soft tissue and shortened structures, restore the range of motion in the left knee joint, increase muscle strength of left lower limb, improve the axial position and spiral screwing of left lower limb reeducate the stereotype of walking, improve static stability of both lower limbs, postural stability and adjust the position of the pelvis. Improvement of muscle strength and static stability is based on results from functional laboratory measurements. I greatly appreciate this cooperation, thanks to which I had the opportunity to work with an interesting and innovative method with which an ACL rupture can be solved and which has enriched me in further practice. The patient praised the cooperation, is satisfied with the results of the joint effort and could actively and without limitation return to life as before the injury.
