Physiotherapy of painful shoulder syndrome

Abstract:

Úvod: Práce se zabývá fyzioterapií u pacientky se syndromem bolestivého ramene - konkrétně parciální ruptury rotátorové manžety. Cíl práce: Cílem bakalářské práce je přispět vhodnými fyzioterapeutickými postupy, koncepty a metodami křešení problematiky fyzioterapie pacientky se syndromem bolestivého ramene - ruptury rotátorové manžety. Metodika: Hlavní část práce byla věnována případové studii 79leté pacientky v penzi. Sběr dat probíhal po dobu fyzioterapeutické intervence v období od 12. 9. 2022 do 13. 10. 2022. S pacientkou bylo realizováno deset terapií s frekvencí dvakrát týdně (každé pondělí a čtvrtek). Na základě vstupního kineziologického rozboru (antropometrie, goniometrie, dynamické vyšetření páteře, vyšetření hlubokého stabilizačního systému a speciálních testů) byl sestaven krátkodobý fyzioterapeutický plán s využitím konceptů a metod: měkké techniky, mobilizace kloubů, relaxační techniky (postizometrická relaxace, postfacilitační strečink, antigravitační relaxace), cvičení s pomůckami (thera-band, overball, gymball, soft míček, masážní míček ježek), cvičení v uzavřených a otevřených kinematických řetězcích, proprioceptivní neuromuskulární facilitace, dynamická neuromuskulární stabilizace a bazální posturální programy. Pátou terapii bylo realizováno kontrolní vyšetření, které bylo zaměřeno na vyšetření rozsahu pohybu, svalové síly, speciálních testů a pohybových stereotypů. Po skončení fyzioterapeutické intervence byl proveden výstupní kineziologický rozbor, na jehož výsledcích byl navržen dlouhodobý fyzioterapeutický plán. Výsledky: Dle výsledků výstupního kineziologického rozboru bylo dosaženo snížení bolestivosti, zvýšení rozsahu pohybu pravého ramenního kloubu do flexe o 15° aktivního pohybu, do abdukce o 20° aktivního pohybu, do zevní rotace o 5° a vnitřní rotace o 10° aktivního i pasivního pohybu. Zvýšila se také svalová síla rotátorové manžety o půl až jeden celý stupeň dle Jandy, a také se zlepšilo vykonávání běžných denních činností (subjektivní hodnocení: otevírání dveří, hygiena, oblékání vrchních dílů oblečení a manipulace předmětů v úrovní hlavy a výše). Závěr: Konzervativní terapie ve formě fyzioterapie kladně ovlivnila bolestivost, rozsah pohybu a svalovou sílu pletence ramenního.

Abstract:

Introduction: The thesis deals with physiotherapy in a patient with painful shoulder syndrome - specifically partial rotator cuff tear. The objective: The aim of the bachelor thesis is to contribute appropriate physiotherapeutic procedures, concepts and methods to solve the problem of physiotherapy of a patient with painful shoulder syndrome - rotator cuff tear. Methodology: The main part of the paper was devoted to a case study of a 79-year-old retired patient. Data collection was carried out during the physiotherapy intervention period from 12 September 2022 to 13 October 2022. Ten sessions were carried out with the patient at a frequency of twice a week (every Monday and Thursday). Based on the initial kinesiological analysis (anthropometry, goniometry, dynamic spinal examination, deep stabilization system examination and special tests), a short-term physiotherapy plan was designed using concepts and methods: soft techniques, joint mobilization, relaxation techniques (post-isometric relaxation, postfacial stretching, antigravity relaxation), assisted exercises (thera-band, overball, gymball, soft ball, hedgehog massage ball), closed and open kinematic chain exercises, proprioceptive neuromuscular facilitation, dynamic neuromuscular stabilization and basal postural programs. The fifth therapy was a follow-up examination that focused on range of motion, muscle strength, special tests and movement stereotypes. At the end of the physiotherapy intervention, an outcome kinesiological analysis was performed and a long-term physiotherapy plan was designed based on the results. Results: According to the results of the output kinesiological analysis, a decrease in pain, an increase in the range of motion of the right shoulder joint into flexion by 15° of active motion, into abduction by 20° of active motion, into external rotation by 5° and internal rotation by 10° of active and passive motion was achieved. There was also an increase in rotator cuff muscle strength of one - half to one full degree according to Janda, as well as an improvement in the performance of activities of daily living (subjective assessment: opening door handles, hygiene, putting on outer clothing and handling objects at head level and above). Conclusions: Conservative therapy in the form of physiotherapy positively affected pain, range of motion and muscle strength of the shoulder girdle.
