Abstract:

Cilem práce je zhodnocení a ověření účinnosti radiosynoviortezy - semiinvazivní konzervativní terapie synovitidy kloubů revmatické ruky.Z výsledků vyplývá výraznější klinický i sonograficky hodnotitelný efekt u RC a MCP kloubů. Výsledky na PIP jsou podprůměrné. RSO je pro MCP a RC plně indikovaná metoda léčby. Pro PIP není metodou první volby.Efekt aplikace RF s lokálním anestetikem je plně srovnatelný s ostatními metodami RSO.Sonografický monitoring při aplikaci je výhodný i pro radiační bezpečnost pacienta a personálu a výrazně snižuje procento radiačních komplikací.Sonografické hodnocení kloubu před a po aplikaci RSO může selektovat pacienty s výraznější revmatickou aktivitou a nasměrovat je k jiné než intraartikulární terapii. RSO je alternativou chirurgického výkonu v rámci prevence,je méně invazivní s nízkým rizikem komplikací a finančně méně náročná.Práce srovnává používané techniky aplikace radiofarmaka. Metoda I, prezentován vlastní postup aplikace radiofarmaka s lokálním anestetikem za ultrasonografické kontroly. Metoda II, shrnutí tří studií z literatury s aplikací radiofarmaka + kortikoid. Metoda III, sumář 4 studií z literatury s aplikací radiofarmaka samostatně. Porovnání jednotlivých metod aplikace a dosažený efekt léčby. Metoda I, aplikace radiofarmaka s lokálním anestetikem za kontroly sonografie, je naprosto rovnocennou s ostatními publikovanými metodami RSO. Lepší klinický efekt dosahuje kombinace radiofarmaka a kortikoidu. Metody II a III zatěžují pacienta rtg zářením.

Abstract:

The aim of the work is to evaluate and verify the effectiveness of radiation synovectomy /RSO/ – semi-invasive conservative therapy of synovitis of joints rheumatoid hand. The results show significant clinical and ultrasonographic appreciable effect on RC and MCP joints. The results of the PIP are below average. RSO is for MCP and RC fully recommended method of treatment. For PIP is not the first choice. The effect of RF application with a local anesthetic is fully comparable with other methods RSO. Sonographic monitoring when the application is also advantageous for the radiation safety of patients and staff and significantly reduces the percentage of radiation complications. Sonographic evaluation of the joint before and after RSO can select for patients with significant rheumatic activity and direct them to other than intra-articular therapy. RSO is an alternative to surgery in the prevention, is less invasive with a low risk of complications and less costly. The work compares used techniques of radiopharmaceutical application. Method I, presents the application procedure of radiopharmaceuticals with local anesthetic under the ultrasound check. Method II, a summary of three studies in the literature with application of the radiopharmaceutical + corticosteroids. Method III, a summary of 4 studies in the literature with application of the radiopharmaceutical itself. Comparison between methods of application and achieved the effect of treatment. The method I, of radiopharmaceuticals with a local anesthetic under the ultrasound check, is absolutely equivalent to the other published RSO methods. Combination of radiopharmaceuticals and corticosteroids achieves better clinical effect. Methods II and III, burden patiens by X-rays.
