Abstract:

Cíl: Cílem rigorózní práce bylo zjistit stupeň psychické zátěže při práci všeobecných sester ve směnném provoze na odděleních intenzivní, standardní a následné péče v NsP Karviná-Ráj, pracoviště Orlová a analyzovat rozdíly ve faktorech ovlivňujících tuto zátěž v závislosti na věku, vzdělání, délce praxe sester a zjištěné výsledky porovnat s výsledky jiných studií. Metodika: Sledovaný soubor tvořilo 118 všeobecných sester pracující ve směnném provoze na lůžkových odděleních intenzivní, standardní a následné péče NsP Karviná-Ráj, pracoviště Orlová. Prostředkem pro získání informací byl standardizovaný Meisterův dotazník. Výzkumné šetření probíhalo v období od 1.9.2013 do 1.1.2014. Pro popis dat byla použita popisná statistika, pro určení rozdílů byly použity statistické testy. Výsledky: Z analýzy dat byl prokázán výsledný první stupeň psychické zátěže na odděleních intenzivní péče ? zde tedy sestry hodnotily svou práci pozitivně. Druhý stupeň zátěže byl rozpoznán na odděleních standardní péče a třetí stupeň psychické zátěže na odděleních následné péče. Mezi nejvíce negativně hodnocené položky patřil pocit časové tísně a dlouhodobá únosnost na všech odděleních. Naopak velmi pozitivně byla hodnocena monotonie. Při analýze rozdílů ve faktorech ovlivňujících psychickou zátěž v závislosti na délce praxe byl zjištěn statisticky významný rozdíl u faktoru I a hrubého skóru. U faktoru I pociťovaly největší zátěž respondentů s nejkratší a nejdelší délkou praxe na daném oddělení. U faktoru II a III nebyly zjištěny statisticky významné rozdíly. Při analýze rozdílů ve faktorech ovlivňujících psychickou zátěž v závislosti na věku nebyl zjištěn statisticky významný rozdíl u žádného faktoru a hrubého skóru. Při analýze rozdílů ve faktorech ovlivňujících psychickou zátěž v závislosti na vzdělání nebyl zjištěn statisticky významný rozdíl v žádném faktoru, ani v hrubém skóru.Při porovnání našich výsledků s výsledky jiných studií jsme zjistily jen mírné rozdíly. Závěr: Práce sestry u lůžka bývá velmi náročnou. Rozdíly vyplývají z typu oddělení, počtu personálu, počtu hospitalizovaných pacientů a jejich zdravotního stavu, vybavení zdravotnických zařízení. To se však může negativně podepsat na zdravotním stavu a pracovním výkonům sestry a často dochází k přenesení potíží i do rodinného života. Aby mohla sestra podávat kvalitní výkony a pomocnou ruku svým klientům, je důležité, aby i ona byla v dobré kondici.

Abstract:

Objective: The aim of the thesis was to determine the degree of psychological stress in nurses working in shifts in intensive, standard and aftercare NsP Karvina-Raj workplace Orlova and analyze differences in factors affecting this stress depending on age, education, length of practice nurses, and the results compared with the results of other studies. Methods: The group consisted of 118 nurses working in shifts on wards intensive, standard and aftercare NsP Karvina-Raj workplace Orlova. Means of obtaining the information was standardized questionnaire Meisterův. The survey covered the period from September 1, 2013 to January 1, 2014. For a description of the data was used descriptive statistics to determine differences has been used statistical tests. Results: From the analysis of data have been demonstrated resulting first degree of psychological stress in intensive care department - the work have been determined positively by the nurses. The second degree of stress was determined in a standard care and the third degree of psychological stress in departments aftercare. Among the most negatively determined items belonged feeling time pressure and long-term bearable level in all departments. Conversely, very positively evaluated the monotony. When has been analyzing a differences in factors which affecting psychological burden depending on the length of practice, the research had determined statistically significant difference in the factor I. and gross score. In factor I. the nurses with the shortest and longest lenght of practice at a department felt the greatest burden. For Factor II and III hasn't been found statistically significant differences. When analyzing differences in factors affecting psychological burden in relation to age was no statistically significant difference in any factor and gross score. When analyzing differences in factors affecting psychological burden in relation to age haven't been found statistically significant difference in any factor and gross skór. When analyzing differences in factors affecting psychological burden depending on education haven't been found statistically significant difference in any factor nor gross score. When we compared our results with other studies, we found only slight differences. Conclusions: The work of nurses at the bedside is very demanding. The differences stem from the type of department, quantity of staff, healthcare facilities also quantity of hospitalized patients and their health. Those facts may have a negative effect on health and work performance of nurses and often is happening that this stress is transferred into family life. The nurse needs to be in a good physical condition that she could provide a quality service and a good care for her patients.
