Minimally invasive treatment of stenosis of ureteropelvic junction (UPJ)

Abstract:

Cílem dizertační práce je stanovení vhodné metody řešení obstrukce pyeloureterální junkce (PUJ) se zaměřením na méně invazivní operační postupy. Autor se ve své práci zabývá reálnými možnostmi použitých méně invazivních metod a hodnotí indikační a vylučující kritéria pro jednotlivé operační techniky. Výzkumná část studuje možnosti detekce a vliv křížící cévy na operační výsledek. Studuje vliv funkce ledviny a velikost ledvinné pánvičky na pooperační výsledek. Hodnotí současná kritéria pro výběr pacientů, která jsou založena na výsledcích endoskopického vyšetření, a dalších zobrazovacích vyšetření. Uvádí relativní výhody a nevýhody antegrádní a retrográdní endopyelotomie, balónové endopyelotomie balónové dilatace, endopyeloplastiky, otevřené a laparoskopické pyeloplastiky. Na základě dosažených výsledků předkládá souhrn indikační kritérií pro volbu jednotlivých metod s cílem o dosažení nejlepších dlouhodobých výsledků.

Abstract:

The ideal treatment for ureteropelvic junction (UPJ) obstruction would be minimally invasive with a low complication and failure rate. Treatment modality would achieve the highest success rate, be capable to correct all forms of UPJ stenosis and remove coexisting renal calculi. The minimally invasive options have become widely accepted because of reduced symptoms and preserved or increased renal function. Endourological techniques were established as minimally alternatives to open pyeloplasty and were introduced with the goal to meet the standard of open dismembered pyeloplasty providing the same long term success rates. With the development of minimally invasive endourologic procedures, either antegrade or retrograde endoluminal approach is used in the treatment of UPJ obstruction. Precise identification of the vascular anatomy of the UPJ may be important for two reasons. Vessel that crosses the UPJ may be cause of the obstruction and might be injured at the time of endopyelotomy. The purpose of study was to evaluate the accuracy of 3D helical CT angiography and its value in surgical planning for the treatment of UPJ. The next objective of study was to estimate advantages and disadvantages of minimally invasive operating methods of stenosis PUJ, antegrade, retrograde or cutting balloon endopyelotomy; and open or laparoscopic pyeloplasty. Authos describes different methods of treatment of UPJ obstruction. Autor presents current criteria for patient selection using endoscopic inspection and adjunctive imaging modalities, and provide a step-by-step discussion of both the antegrade and retrograde endoscopic approaches to management of this disease.
