TheraSuit concept in the treatment of clients with Dandy-Wolker syndrome

Abstract:

Cíl: Cílem bakalářské práce je přispět k problematice fyzioterapie u klientů s Dandy-Walker syndromem v terapii TheraSuit method?. Metodika:Ke zpracování kazuistiky bylo třeba, ve spolupráci s pacientkou B.N. odebrat anamnézu, vypracovat kineziologické vyšetření a neurologické vyšetření fyzioterapeutem. Dále bylo provedeno hodnocení funkčních úchopů ruky a hodnocení funkční nezávislosti dle testu FIM. Na základě těchto vyšetření byly určeny cíle krátkodobého fyzioterapeutického plánu. Ten se realizoval pomocí TheraSuit method? v podobě 2 měsíců. Po měsíční terapii jsem provedla kontrolní vyšetření s ohledem na sledované parametry(hodnocení funkce ruky, orientační testy mozečku,vyšetření reflexů). Po druhém měsíci terapie jsem provedla výstupní vyšetření, kde byly použity identické metody. Na základě odebraných hodnot byl stanoven dlouhodobý fyzioterapeutický plán. Výsledky:Ve výsledných parametrech aspekce došlo k prohloubení skoliotické křivky a postavení LDK do dorsální flexe. Ve vyšetření spasticity došlo ke snížení hodnot a to z 3 na 2 dle Ashworthovy škály u mm.adduktores kyčelních kloubů. V parametrech neurologického vyšetření došlo ke změnám ve změně hodnot z 1 na 2 u reflexu šlachy Achilovy a medioplantárního na DKK. U PHK došlo ke změně hodnot z 0 na 1 u všech myotatických reflexů. Na LHK pouze u reflexu tricipitového a bicipitového. V oblasti parametrů funkčních úchopů ruky došlo ke zlepšení na PHK, která byla schopna úchopu špetkou. V hodnocení funkce nezávislosti dle testu FIM došlo ke změnám v kategorii osobní péče z bodu 1 na bod 2. Klientka dokázala vypomáhat při oblékání horní poloviny těla a při přesunech. Závěr: Přestože došlo u některých parametrů ke snížení hodnot. Myslím, že metoda TheraSuit? je vhodná pro jedince s touto diagnosou. Hlavního cíle práce, bylo dle mého uvážení dosáhnuto.

Abstract:

Goal:The main goal of this theses is to contribute to the problem of Dandy-Walker syndrome patients in TheraSuit method? therapy. Method:For the case study, a medical history, a kinesiological examination, a neurological examination had to be carried in cooperation with patient B.N.. It was also evaluated functional hand grip and evaluation of functional independence in accordance with the FIM test. On the bases of this examination were determined the goal of the short-term physiotherapeutic plan. This was done using TheraSuit method? for 2 months. After monthly therapy, I performed check-up with respect to the parameters monitored(assessment of hand function, cerebellum orienteering tests, reflex examination). After the second month of therapy was done final examination, where applicable identical methods. On the basis of the collected values, the long-term physiotherapeutic plan was established. Results:In the resulting of the parameters of the aspect, the skoliotic curve was emphasized, the position of the left lower limb in the dorsal flexion. Neurological examination parameters have changed in reflexes of achil and medioplanar reflexes, also in hypermetry and diadochokineses tests in the sense of the improvement of these values. In the area of hand grip parameters, the gross and fine motor on the upper right arm improved. In the functional independence accordance with the FIM test, there were changes in the category of personal care. Conclusion: All the tasks that were entered at the beginning the bachelor's thesis were fulfilled. The partial goals that we wanted achieve with the patient were achieved the most. There has been overall improvement in client activity. The main aim of the bachelor thesis was, in my opinion, achieved.
