Children with brachial plexus palsy from the perspective of occupational therapy

Abstract:

Cíl: Cílem mé bakalářské práce je informovat o možnostech využití senzorické integrace v rámci ergoterapie u dětí s parézou plexus brachialis. Dalším cílem je zmapovat a ukázat tyto možné širší souvislosti ve smyslovém zpracování u dětí s parézou brachiálního plexu a navrhnout ergoterapeutické možnosti a strategie pro zlepšení. U kazuistiky je zaznamenáno kompletní vyšetření, stanovení terapie i její výsledky a následná doporučení ve formě dlouhodobého rehabilitačního plánu. Metodika: Hlavním úkolem této části práce bylo vypracování kazuistiky pacienta a otestování vzorku pacientů testem SIPT s parézou plexus brachialis. Na testování docházeli vhodně zvolení pacienti ambulantně, v námi zvoleném termínu. Celkem bylo otestováno 8 dětí. Testování probíhalo v různých hodinách a dnech po dobu zhruba 6090 minut. Přístup k pacientům byl individuální vzhledem k jejich povaze a postižení. Pro vypracování kazuistiky jsem při prvním setkání získala anamnézu rozhovorem se zákonnými zástupci a vstupní ergoterapeutické vyšetření na základě kterého jsem si stanovila krátkodobý ergoterapeutický plán a provedla terapie pomocí vhodně zvolených ergoterapeutických metod. Poslední den terapie jsem provedla výstupní ergoterapeutické vyšetření s dlouhodobým ergoterapeutickým plánem, kterého by se měl pacient dále držet při rehabilitaci. Výsledky: Na základě všech zjištěných údajů, klinického pozorování a testování testem SIPT bylo možné zhodnotit smyslové vnímání, kde se objevily určité poruchy, převážně v oblasti propriocepce, vestibulárního a taktilního čití a také v oblasti hmatového, sluchového a zrakového vnímání. Závěr: Test SIPT odhalil poruchu senzorického zpracování u všech 8 respondentů, tím se prokazuje jeho využitelnost pro ergoterapeuty. Vidím v rozvíjejícím se přístupu senzorické integrace nové možnosti, které nám poskytují příležitost dívat se na jedince a jeho poruchy jiným pohledem a z jiného úhlu pohledu tyto problémy i řešit. Cíl práce byl tedy splněn.

Abstract:

Aims: The aim of my bachelor thesis is to inform about the possibility of using sensory integration in the framework of ergotherapy in children with plexus brachialis paresis. Another aspect is to map and show these possible broader connections in sensory processing in children with cerebral plexus and suggest ergotherapeutic options and strategies for improvement. The case report requires a complete examination, determination of the therapy and its results and subsequent recommendations in your long-term rehabilitation plan. Methods: Collection with plexus brachialis paresis. Appropriately chosen persons were outpatient at the time chosen by us. A total of 8 children were tested. Testing awaits on different days and waits 60-90 minutes after the stay. The approach to patients was an individual view of their nature and disability. To elaborate the case report I am in my meeting with the medical history of the interview with legal representatives and when submitting ergotherapy examinations, based on, evaluation and evaluation of the ergotherapy plan and proven therapy using appropriately obtained ergotherapy methods. On the last day of therapy, I gave an ergotherapy examination with a long-term ergotherapy plan that the patient should continue to hold during rehabilitation. Results: Based on all available data, clinical observation and test testing, where errors occurred in the areas of proprioception, vestibular and tactile sensation and in the field of tactile, auditory and visual perception. Conclusion: The SIPT test revealed a sensory disorder in all 8 respondents, proving its usability for occupational therapists. We see how to look at sensory integrations of new possibilities that help us to look at one of them and its failures from the point of view, or from the point of view of these problems and solve them. The aim of the work was thus fulfilled.
