Physiotherapy of stress fracture in the area of ankle joint in runners

Abstract:

Cíle: Tato bakalářská práce má za cíle prohloubit teoretické znalosti a praktické dovednosti související s diagnózou únavové zlomeniny v oblasti hlezenního kloubu u běžců. Metodika: V bakalářské práci byla zpracována kazuistika pacienta po únavové zlomenině v oblasti hlezenního kloubu na pravé noze. Proběhlo celkem deset terapií, při kterých byly použity různé metody a pomůcky. Na základě vstupního kineziologického rozboru byl navrhnut krátkodobý fyzioterapeutický plán. Zpočátku byla terapie zaměřena hlavně na odstranění otoku a bolesti pomocí míčkové facilitace, lymfotapingu, uvolnění měkkých tkání a pasivní pohyb. Později byla terapie zaměřena na ovlivnění plosky nohy, aktivně asistovaný a aktivní pohyb. Při terapiích byly mobilizovány klouby kotníku dle Lewita, pacient byl poučen o korekci chůze o berlích a modifikaci chůze. Byly využity pomůcky jako masážní válec, thera-band či bosu. Terapie dále zahrnovaly cviky z konceptu PNF, Brügger koncept a také SMS. Od první terapie měl pacient také magnetoterapii na pravém kotníku a čtyři procedury laseru. Na závěr byly porovnány výsledky měření, pacient byl instruován, jak předejít dalším únavovým zlomeninám a byl zhodnocen efekt terapie. Výsledky: Pacient se vrátil zpět k běhání díky odstranění bolesti, dosažení plného rozsahu pohybu, zvýšení svalové síly a stabilizaci v hlezenním kloubu.

Abstract:

Purpose: This bachelor thesis aims to deepen theoretical knowledge and practical skills related to the diagnosis of stress fracture in the ankle joint in runners. Methodology: In the bachelor's thesis, a case report of a patient after a stress fracture in the ankle joint on the right leg was processed. There was a total of ten therapies, in which we used various methods and aids. Based on the initial kinesiological analysis, a short-term physiotherapy plan was proposed. Initially, the therapy focused mainly on the elimination of swelling and pain through ball facilitation, lymph taping, soft tissue relaxation, and passive movement. Later, the therapy focused on affecting the arch of the feet, active-assisted and active movement. During the therapies, ankle joints were mobilized according to Lewit's method, the patient was instructed to correct gait with forearm crutches and gait modification. Aids such as a massage roller, thera-band, or bosu were used. Therapies also included exercises from the concept of PNF, the Brügger concept, and SMS. Since the first therapy, the patient has also had magnetotherapy on the right ankle and four laser procedures. Finally, the results of the measurements were compared, the patient was instructed on how to proceed with further stress fractures, and the effective therapy was evaluated. Results: The patient returned to running by removing the pain, performing the full range of motion, increasing muscle strength, and stabilizing in ankle joint.
