Rehabilitation of daily motor activities: passive stretching technique compared to CRS (Contract Relax Stretch) method.

Abstract:

This thesis is entitled ?Rehabilitation of the daily motion: passive stretching technique compared to the CRS method (Contract-Relax Stretch)?: it is an orthopaedic and experimental thesis, which means that it is based on data and measures I collected during my practical two-months apprenticeship. My aim was to make a comparison between two stretching techniques after a surgical operation of a two-compartment knee prosthesis. The comparison is made both in terms of recovery of the articular (ROM) and in terms of speed of the patient in achieving the autonomy which is necessary for his discharge. In facts, in any orthopaedical or traumatological injury both the articular recovery (ROM) and the muscular strength have a fundamental role. Given that the muscles involved are ?shortened?, one of the techniques to be used is stretching, which allows to regain the physiological muscular length needed to do any daily activity. There are several stretching techniques: dynamic, active static, passive static and CRS (Contract-Relax Stretching). This survey is rotates around two techniques: the passive static, in which muscles are lengthened through the help of a partner, and the CRS method; this system is based on a muscular contraction of 10-15 seconds, followed by 5 seconds of relaxing and a further stretching. Apart from affecting the muscle spindle, stretching lengthens any structure involved: in particular it subjects the nerves to a pressure which sometimes can be excessive and compromises the correct transmission of the nerve impulse. The CRS method aims to solve this problem of excessive compression and stretching of the nervous structures, preserving its transmission?s physiology to avoid any relapse of injuries, although it keeps the effectiveness and the schedule of the rehabilitation of the affected area. This stretching combines an explicit effect, which can be seen in the muscular recovery, and an implicit one, consisting of an enhancement of the proprioception in the involved area. The sample of patients was divided into two uniform groups, in which the age was included between 67 and 77, and each group was given a protocol they had to follow during the convalescence: one of them did the passive static stretching and the other the CRS method. Twice a week, I measured the articular increase in the push-up of the knee through an orthopaedic goniometer, as well as the corresponding numerical value through the muscular grading. At the moment of the patient?s discharge data were organized in charts, and subsequently I calculated the Riabilitazione del gesto quotidiano: tecnica dello stretching passivo a confronto con la metodica CRS (Contract Relax Stretch ) 13 average ROM for each group and session. Once I finished collecting data and registering averages and standard deviations, I was able to make a comparison between the groups. The comparison was made both through the observation of the numerical values and a histogram, which shows how much a technique can be more effective than the other over the sessions. A further short analysis concerns the values of muscular grading during the convalescence period. After analyzing these values, the result is that the CRS method allows patients to regain health more quickly than the static stretching technique. Nevertheless, even though the passive technique takes more time, it reaches the same results: patients of the CRS group were released before the estimated date only in two cases. Both the stretching techniques turn out to be effective, and their use is at the therapist?s and the patient?s discretion, given that not all the patients are able to do the CRS technique.

bstract Questa tesi dal titolo ?Riabilitazione del gesto quotidiano: tecnica dello stretching passivo a confronto con la metodica CRS (Contract Relax Stretch)? č una tesi a carattere ortopedico e di tipologia sperimentale, vale a dire con dati e misure raccolti sul campo nei due mesi di tirocinio a disposizione. Lo studio da me effettuato ha lo scopo di confrontare tra loro due tecniche di stretching in una post operazione di protesi bicopartimentale di ginocchio. Il confronto avviene sia in termini di recupero del ROM articolare sia in termini di celeritŕ da parte del paziente nel raggiungere l?autonomia necessaria alla dimissione dalla struttura. In qualsiasi lesione ortopedica o traumatologica infatti, all?interno del progetto riabilitativo, risulta di fondamentale importanza sia il recupero articolare (ROM) che la forza muscolare. Una delle tecniche utilizzate, poiché la muscolatura interessata risulta ?accorciata?, č lo stretching, il quale permette di recuperare la lunghezza muscolare fisiologica per svolgere qualsiasi attivitŕ quotidiana. Esistono numerose tecniche di stretching come: dinamico, statico passivo, statico attivo e CRS (contract relax stretch). Questo studio si č concentrato su due tipologie, in particolare quello statico passivo (nel quale l?allungamento del muscolo avviene mediante l?aiuto di un partner); e il CRS (sistema basato su una contrazione del muscolo per 10-15 secondi, seguita da 5 secondi di rilassamento ed un successivo allungamento). Oltre ad avere un effetto sul fuso muscolare, lo stretching pone in allungamento qualsiasi struttura interessata, in particolare sottopone i nervi ad una tensione talvolta eccessiva, che non permette piů la corretta trasmissione del segnale nervoso. Il metodo CRS cerca di ovviare a questo problema di compressione e stiramento eccessivo delle strutture nervose, rispettandone la fisiologia di trasmissione per evitare recidive di infortuni pur mantenendo l?efficacia e la tempistica del recupero nella lesione interessata . Lo stretching combina sia un effetto estrinseco, visibile dal recupero articolare, sia un effetto intrinseco di aumento della propriocezione del distretto interessato. Il campione di pazienti č stato diviso in due gruppi omogenei di etŕ compresa tra i 67 e i 77 anni di etŕ, ai quali ho affidato un protocollo ciascuno da seguire durante il corso del periodo di degenza. Un gruppo avrebbe quindi eseguito lo stretching statico passivo ed un gruppo il CRS. Ogni settimana, per due volte grazie all?utilizzo di un goniometro ortopedico misuravo l?incremento articolare della flessione di ginocchio e con l?aiuto del grading muscolare il corrispondente valore Riabilitazione del gesto quotidiano: tecnica dello stretching passivo a confronto con la metodica CRS (Contract Relax Stretch ) 11 numerico. Alla dimissione del paziente i dati erano raccolti in tabelle e in seguito avveniva il calcolo della media dei ROM divisa per gruppi e per seduta. Una volta ultimata la raccolta dati e la registrazione delle medie e delle deviazioni standard si č potuto eseguire il confronto fra i due gruppi. Il confronto č avvenuto sia tramite osservazione dei valori numerici che tramite grafico istogramma, dove nel corso delle sedute appare quanto una tecnica abbia guadagnato rispetto ad un'altra. Un?ulteriore breve analisi riguarda i valori numerici del grading muscolare durante il periodo di degenza. Dall?analisi dei dati emerge che la tecnica CRS di Kabat pone il paziente nelle condizioni di recuperare piů velocemente rispetto alla tecnica di stretching statico. Tuttavia anche se la tecnica passiva risulta piů lenta entrambi arrivano al medesimo obiettivo, in soli due casi infatti il paziente dell?altro gruppo č stato dimesso preventivamente da quanto progettato. Risultano quindi entrambi efficaci le tecniche di allungamento ed il loro utilizzo va a discrezione del terapista e del paziente, dato che non tutti i pazienti sono in grado di effettuare la tecnica CRS.
