Comparison of physiotherapy protocols following surgery for meniscal knee injury: a clinical case

Abstract:

The knee joint is one of the most important within the human body, responsible for performing various functions such as standing upright gaze, walking and movement. In the context of the anatomo-structural complex of the knee, it is essential to mention the menisci: fibro-cartilaginous structures that increase the articular surface between the femur and tibia and deals with distributing the ascending and descending forces that weigh on our body and of which the knee is involved the crossroads. The objective of the following thesis will be to go and observe in detail this articulation both from an anatomical, biomechanical and muscular point of view; in fact, its stability is guaranteed by numerous muscles and above all by the intrinsic function performed by the ligament capsule system. The attention on the role played by the menisci in the stabilization of the joint and on what can be the traumatic/degenerative and associated injuries. Such injuries from an epidemiological point of view are much more frequent in subjects who practice sports due to the heavy loads to which the knee is often subjected, such as for example the forces generated during a repeated athletic gesture or during an impact trauma. To these are added the degenerative lesions associated with the arthritic processes typical of older patients, however it is essential not to underestimate the extent of damage at the meniscal level as the lesion can evolve into chronic forms of wear and arthritic damage. Nowadays some innovative methods have been discovered with the aim of preserving the meniscal structure by virtue of the role it plays; these will include the intervention of total or partial meniscectomy in arthroscopy and the saturation in which the injured segment will not be removed but the injured meniscus will be sutured. Each patient must necessarily undergo a personalized and detailed rehabilitation path structured by his / her therapist composed of walking rehabilitation exercises, strengthening of muscle tone, massage therapy, physical therapy of the return to the performance of sporting activities and of one's daily life.

Abstract:

L'articolazione del ginocchio ? una delle pi? importanti presenti all'interno del corpo umano, deputato allo svolgimento di svariate funzioni quali lo stare in posizione eretta, il camminare e il muoversi. Nell'ambito del complesso anatomo-strutturale del ginocchio ? fondamentale citare i menischi: strutture fibro-cartilaginee che vanno ad aumentare la superficie articolare tra femore e tibia e consentono di distribuire in maniera adeguata le forze ascendenti e discendenti che gravano sul nostro corpo e di cui il ginocchio ne rappresenta il crocevia. L'obiettivo della seguente tesi sar? quella di andare ad analizzare nel dettaglio questa articolazione sia da un punto di vista anatomico, biomeccanico e muscolare; infatti la sua stabilit? viene garantita da numerosi muscoli e soprattutto dalla funzione intrinseca svolta dal sistema capsulo legamentoso. Si porr? l'attenzione sul ruolo svolto dai menischi nella stabilizzazione dell'articolazione e su quelle che possono essere le lesioni traumatiche/degenerative ad essi associate. Tali lesioni dal punto di vista epidemiologico sono molto pi? frequenti nei soggetti che praticano attivit? sportiva a causa dei forti carichi a cui il ginocchio spesso viene sottoposto, quali ad esempio le forze che si generano durante un gesto atletico ripetuto o durante un trauma da impatto. A queste si aggiungono le lesioni degenerative associate a processi artrosici tipiche di pazienti pi? anziani, tuttavia ? fondamentale non sottovalutare l'entit? del danno a livello meniscale in quanto la lesione pu? evolvere in forme croniche di usura e conseguente danno artrosico. Al giorno d'oggi sono stati scoperti alcune metodiche innovative aventi come finalit? il preservare la struttura meniscale in virt? del ruolo da essa svolta; tra questi verranno citati l'intervento di meniscectomia totale o parziale in artroscopia e quella saturazione in cui non si andr? a rimuovere il segmento lesionato ma si proceder? alla sutura del menisco leso. Ciascun paziente dovr? successivamente sottoporsi ad un percorso riabilitativo personalizzato e dettagliatamente strutturato dal proprio terapeuta composto da esercizi di rieducazione alla deambulazione, rinforzo del tono muscolare, massoterapia, terapia fisica del ritorno allo svolgimento di attivit? sportiva e della propria vita quotidiana.
