Cervical sprain: comparison of two therapeutic approaches.

Abstract:

TRAUMA DISTORSIVO CERVICALE: DUE APPROCCI TERAPEUTICI A CONFRONTO 8 ABSTRACT Whiplash is an injury that commonly occurs as a result of a road accident and mainly involves the cervical spin but may also present associated disorders. At the beginning of this thesis" Trauma cervical sprain two therapeutic approaches compared" I will explain the anatomical complexity of the cervical spine for the presence of numerous ligaments, for the action of muscles on different levels for the articulation, in particular of 'atlas first cervical vertebrae and of the axis That are two vertebrae very different from each other for the structure. Afterwards I'll discuss the dynamic and mechanisms of the damaging of the whiplash highlighting what are the main risks and damages in which you can find your self, for example, the straightening of the cervical spine which is very frequent, and what are the techniques of research for the diagnosis. The reference to the legal -insurance aspect is inevitably given the numerous problems concerning the correct diagnosis of whiplash by the coroner/ medical examiner in order to recognize the cases in which they talk about true pathology from the simulation cases for the correct refund of medical expenses. There will be carried out an experimental test on patients that reflect the criteria of inclusion and exclusion afterwards described which are divided into two groups. A group will be treated with physiotherapy ( pompage, active exercises and isometrics),tecar and the other with the same physical therapy and laser. The assessments of pain by VAS and the joint mobility through a tape measure has shown that the most effective treatment was the one with the tecar for four out of seven parameters. It had significant results flexion, extension, left and right inclinations, while the left and right rotations and the VAS didn't have significant results for the statistical calculation made by the "t" test of Student. In both groups, however, improvements have been made following the therapies. This comparison was made possible thanks to the use of the test just mentioned that leads us to say which values for the results obtained are due to chance.

Abstract:

Il colpo di frusta č un trauma che si verifica comunemente in seguito a un incidente stradale e coinvolge principalmente il rachide cervicale ma puň presentare anche dei disturbi associati. All?inizio di questa tesi ?Trauma distorsivo cervicale: due approcci terapeutici a confronto? spiegherň la complessitŕ anatomica del rachide cervicale per la presenza di numerosi legamenti, per l?azione di muscoli su piani differenti e per l?articolaritŕ, in particolare dell?atlante e dell?epistrofeo che sono due vertebre molto diverse per la conformazione. Successivamente tratterň la dinamica e i meccanismi lesivi del colpo di frusta evidenziando quali sono i principali rischi e danni a cui si va in contro, ad esempio la rettilineizzazione del tratto cervicale il quale č molto frequente, e quali sono le indagini strumentali per la diagnosi. Un riferimento all?aspetto assicurativo-legale č inevitabile visti i numerosi problemi riguardo la corretta diagnosi di colpo di frusta da parte del medico legale al fine di riconoscere i casi in cui si parla di vera patologia dai casi di simulazione per il corretto rimborso delle spese mediche. Verrŕ svolta una tesi sperimentale su pazienti che rispecchiano dei criteri di inclusione e di esclusione successivamente descritti i quali sono divisi in due gruppi. Un gruppo sarŕ trattato con la fisioterapia (pompage ed esercizi attivi e isometrici) e la tecar e l?altro con la stessa fisioterapia e il laser. Le valutazioni del dolore tramite la scala VAS e quella della mobilitŕ articolare tramite un metro a nastro ci hanno indicato che il trattamento piů efficace č stato quello con la tecar per quattro parametri su sette. Č risultata significativa la flessione, l?estensione, le inclinazioni destra e sinistra, mentre le rotazioni destra e sinistra e la VAS non sono risultate significative per il calcolo statistico effettuato tramite il test ?t? di Student. In entrambi i gruppi perň si hanno dei miglioramenti in seguito alle terapie. Questa comparazione č stata possibile grazie all?utilizzo del test appena citato che ci porta ad affermare per quali valori i risultati ottenuti siano dovuti al caso.
