Identifikace a kvantitativní monitorování antigen-specifických T lymfocytů u onkologických pacientů

Abstract:

Imunoterapie má dnes významné postavení mezi strategiemi léčby nádorových onemocnění. Pokrok v poznatcích nádrové imunologie umožnil rozpracovat postupy, které staví na mechanismech protinádorové imunity. Po průkopnických studiích v oblasti nespecifické odpovědi se výzkumné úsilí zaměřilo na specifickou protinádorovou imunitu. Nádorová buňka představuje terč imunitní odpovědi, protože na svém povrchu nese nádorové antigeny. Hlavní úlohu v rozpoznání těchto antigenů má cytotoxický T lymfocyt. Cytotoxický T lymfocyt je také svými efektorovými mechanismy schopen rozpoznanou nádorovou buňku eliminovat. Adoptivní buněčná imunoterapie (ACT adoptive cell transfer) potencuje tuto tumor specifickou imunitní reakci, která je v podmínkách in vivo slabá a neúčinná. Tumor specifické cytotoxické T lymfocyty se pro ACT terapii nejdříve separují a potom expandují do klinicky využitelného množství. Poté jsou intravenózně podány pacientovi, kde zahájí účinou protinádorovou reakci. Tento léčebný přístup je postupně zaváděn, nicméně podrobné zkoumání chování přenesených buněk je stále předmětem výzkumu. Pro studium chování přenesených buněk je nezbytné tyto buňky jednoznačně identifikovat. Tato práce se zabývala zavedením metody determinace T lymfocytů na základě sekvence CDR3 jejich podjednotky TCR receptoru. Metoda byla úspěšně zavedena a byly získány první výsledky na vzorcích od pacientů s maligním melanomem. Práce potvrdila představu existence imunodominantních klonů tumor specifických T lymfocytů. Díky možnosti jednoznačně identifikovat tyto klony tak práce otevřela prostor pro jejich monitorování v dalších studiích.

Abstract:

Recently, immunotherapy has been one of the leading approaches to treating oncological diseases. Advances in the tumor immunology have provided the development of methods which are based on antitumor immunity mechanisms. Frontier studies in nonspecific immunity area have been followed by research endeavour targeted on specific antitumor immunity. Tumor cell constitutes target for immunity response for its tumor antigen exposition on its cell surface. Cytotoxic T lymphocyte has the central role in the recognition of these tumor antigens. Cytotoxic T lymphocyte is able to eliminate recognized tumor cell due to its effector mechanisms. Adoptive cell transfer immunotherapy (ACT) potences this tumor-specific immunity reaction which is weak and inefficient in vivo. For ACT therapy tumor-specific T lymphocytes are separated at first and then they expand to amounts sufficient for clinical purposes. Then they are intravenously administered and in the tumor site they start efficient antitumor response. This treatment approach is establishing progressivelly, but detailed investigation of transfered cells behavior is still a subject of research. For studying these cells it is necessary to indentify them unambiguously. The aim of this study was the introduction of the method of the determination of the T lymphocytes basis on the CDR3 sequence of beta-subunit of T cell receptor. The method was succesfully introduced and first records have been achieved from malignant melanoma pacients specimens. In this work the notion of the existence of immunodominant clones of tumor-specific T-lymphocytes was confirmed. Thanks to the possibility to identify these clones there is room for monitoring them in subsequent studies.
