Abstract:

Obezita je spojená se zhoršenými kognitivními schopnostmi a dysregulací nálad, ale mechanismy, které jsou základem těchto vztahů, nejsou dobře známy. Cílem disertační práce bylo posunout doposud omezené poznání na základě korelačních a průřezových studií kognitivních funkcí u obezity a zjistit, zda mohou být změny strukturálních vlastností šedé hmoty, kognitivních poruch a nálady vratné. Nejprve jsme pomocí meta-analýzy strukturálních oblastí mozku identifikovali oblasti, které jsou pro mírnou kognitivní poruchu (MCI) a velkou depresivní poruchu (MDD) specifické, a které naopak sdílené, a mohly by tak predikovat společné rysy onemocnění. Ve druhé studii jsme zkoumali, zda snížení tuku po bariatrické operaci může vést nejen ke zlepšení kognitivních funkcí, ale i nálady. V poslední studii jsme testovali možný vliv tělesného tuku na zrychlení biologického stárnutí u mladých dospělých. Porozumění vztahům mezi metabolickými změnami souvisejícími s hmotností a změnami ve struktuře mozku, kognici a depresivní symptomatologii může do budoucna zlepšit naše chápání patofyziologie obezity, změn po bariatrické operaci a jejich souvislosti s duševním zdravím.

Abstract:

Obesity is associated with impaired cognitive abilities and mood dysregulation, but the mechanisms underlying these relationships are not well understood. In this dissertation, we sought to advance previously limited information from existing correlational and cross-sectional studies of cognitive function in obesity and determine whether changes in structural properties of grey matter, cognitive impairment, and mood may be reversible. In the first study, we performed a meta-analysis of structural magnetic resonance studies to identify brain regions, which are specific for mild cognitive impairment (MCI) and major depressive disorder (MDD) and which are shared between these two disorders and thus might elucidate common underlying mechanisms. In the second study, we examined the effect of bariatric surgery on mental health and tried to determine whether the reduction of fat after bariatric surgery could lead to improved cognitive function and mood. In the last study, we tested the possible effect of body fat on accelerated biological aging in young adults. Better understanding of the relationships between weight-related metabolic changes and changes in the brain structure, cognition, and depressive symptomatology may improve our understanding of the pathophysiology of obesity, changes after bariatric surgery, and their impact on mental health.
