Variability of foot muscles tendons in relation to the onset of pathological changes

Abstract:

Úvod. Velmi podceňovanou oblastí v klinické praxi jsou bolesti laterální strany nohy, které jsou při podrobnějším zkoumání velmi časté. Jejich příčina bývá bohužel většinou nerozpoznána. Mezi faktory související s bolestí v této lokalitě může patřit variabilita anatomických struktur. Z tohoto důvodu byla v předkládané dizertační práci sledována variabilita v oblasti průchodu a úponu šlach perineálních svalů na laterální straně nohy. Metody. Pro potřeby studie bylo zhotoveno a hodnoceno 30 anatomických preparátů dolních končetin (18 mužů, 12 žen) deponovaných na Anatomickém ústavu LF MU. Všechny vzorky byly fixovány a uchovávány podle standardních metod. Druhý hodnocený soubor zahrnoval 768 nativních RTG snímků nohou v nemocnici Vamed Mediterra Mostiště. Hodnocený soubor sestával ze 415 mužů (54 %) a z 353 žen (46 %). Výsledky. Jasná přítomnost os peroneum na RTG snímcích byla zjištěna ve třech případech – 10 % z 30 preparátů. Z osteometrické analýzy os cuboideum, I. metatarzu, os cuneiforme mediale a calcanea byl ve vztahu k přítomnosti os peroneum signifikantní rozdíl zjištěn v délce os cuneiforme mediale a hraničně v délce žlábku na os cuboideum. Klasický úpon m. peroneus longus ve tvaru písmene V byl přítomen v 9 % případů, s 23 % tzv. minor varianty na os cuneiforme mediale. Při zohlednění přítomnosti os peroneum byl nalezen trend vyšší přítomnosti os peroneum vzhledem k typu II úponu m. peroneus longus. Z osteometrických dat byl vztah úponu m. peroneus longus asociován s vzdáleností eminentia retrotrochlearis a trochlea peronealis, hraničně s délkou prvního metatarzu a calcanea. V hodnocení přítomnosti os peroneum v závislosti na délce metatarzů byl nalezen rozdíl v prevalenci prvního metatarzu I+ oproti ostatním variantám se sedminásobně vyšším výskytem v přítomnosti os peroneum. V přítomnosti os peroneum byla nalezena trojnásobně vyšší přítomnost duplexní sezamské kůstky v oblasti art. metatarzophalangealis V a více než šestinásobně vyšší výskyt sezamské kůstky os tibiale externum. Závěr. Přesto, že k bolesti laterální strany nohy přispívá mnoho faktorů, nebyl jasně prokázán vztah bolesti a variability některé z anatomických struktur.

Abstract:

Background. The lateral foot pain, the most likely very common, is underestimated in the clinical practice. The cause is not usually detected. Factors associated with the pain in this area may include variability of anatomical structures. For this reason, the present dissertation focused on this issue. Variability was observed in the pass and attachments of the peroneal muscles tendons on the lateral side of the foot. Methods. For the purposes of this study 30 anatomical specimens of the lower extremities (18 men, 12 women) deposited at the Department of Anatomy LF MU were prepared and evaluated. All samples were fixed and stored. Second evaluated file included 768 native radiographs of the foot collected in a hospital Vamed Mediterra Mostiště. Evaluated group consisted of 415 men (54%) and 353 women (46%). Results. Clear presence os peroneum on radiographs was found in three cases – 10 % of 30 specimens. From the osteometric analysis of selected parameters (os cuboideum, first metatarsal, os cuneiforme mediale and calcaneus), significant relation to the presence of os peroneum was observed in the length of os cuneiforme mediale, and marginally in the length of the groove on the os cuboideum. Classical insertion of m. peroneus longus was present in 93 % of cases, with 23 % of the so-called minor variant on the os cuneiforme mediale. Regarding the presence of os peroneum, a trend to higher presence of the type II insertion of m. peroneus longus was observed. From the osteometric parameters, the insertion of m. peroneus longus was associated with distance between eminentia retrotrochlearis and trochlea peronealis, marginally with the length of the first metatarsal and calcaneus. Evaluating the presence of os peroneum in relation to the length of metatarsal, the sevenfold higher incidence in the prevalence of first metatarsal I+ compared to other variants in the presence os peroneum was found. Furthermore, in the presence of os peroneum, the three-fold higher prevalence of metatarzophalangeal V duplex sesamoid bones and more than six-fold higher incidence of os tibiale externum was found. Conclusion. Despite the fact that many factors contribute to the onset of the lateral foot side pain, the relationship between the pain and variability of selected anatomical structures was not clearly established.
