Targeted temperature management after CPR

Abstract:

Tato bakalářská práce pojednává o cílené kontrole teploty u pacientů po netraumatické srdeční zástavě, kteří po úspěšné resuscitaci a návratu spontánní cirkulace zůstávají komatózní. Tyto terapeutické postupy jsou již od roku 2005 součástí doporučených postupů poresuscitační péče, i když způsoby provádění a hodnoty cílové teploty prodělaly od této doby určitý vývoj. Teoretická část bakalářské práce popisuje fyziologii a patofyziologii tělesné teploty a její regulaci, způsoby jejího měření a úskalí, která měření této vitální hodnoty provázejí. Dále popisuje historický vývoj užití metody terapeutické hypotermie, způsoby jejího provádění, indikace i kontraindikace této metody, popisuje patofyziologické procesy, které jsou podkladem jejího léčebného efektu, a popisuje také nežádoucí účinky, které provázejí tento léčebný postup. Praktická (výzkumná) část bakalářské práce je rozdělena do 2 částí. První část si klade za cíl popsat praktické provádění této léčebné metody na interní jednotce intenzivní péče. Cílem je též zjistit, jakým způsobem probíhá monitorace teploty u pacientů po srdeční zástavě v praxi, jaké metody k cílené kontrole teploty jsou využívány a s jakou úspěšností je na konkrétním pracovišti dosahováno vytyčených cílů. Pro tuto analýzu byl zvolen soubor pacientů, hospitalizovaných na interní JIP Fakultní nemocnice Plzeň po zástavě oběhu s úspěšnou resuscitací v období od 1.1.2016 do 1.1.2017. Jedná se o retrospektivní práci, souhrnně zpracovávající anonymní data získaná analýzou lékařské dokumentace. V závěrečném hodnocení práce porovnává dosažené výsledky s publikovanými daty z jiných pracovišť. Podrobně zkoumá výše uvedený soubor pacientů, jejich vstupní rytmus a základní příčiny, které k srdeční zástavě vedly. Dále analyzuje mortalitu a neurologické výstupy u této skupiny pacientů a porovnává výsledky s literárními daty z jiných pracovišť. Druhá část výzkumné práce je zaměřena na subjektivní hodnocení provádění cílené kontroly teploty ošetřujícím personálem. Tato část výzkumu byla provedena formou dotazníkového šetření s cílem zjistit, jak zatěžující je pro ošetřující personál provádění této metody, v čem ošetřující personál vidí hlavní problém při používání této metody a v neposlední řadě bylo zjišťováno, co by ošetřující personál na aplikaci tohoto postupu chtěl změnit či co mu pro provádění tohoto postupu schází.

Abstract:

This bachelorś thesis deals with targeted temperature management in patients after nontraumatic cardiac arrest, who remain nonresponsive after return of spontaneous circulation. This terapeutic method is part of guidelines for post-resuscitation care since 2005, although the way of performing this procedure and targeted temperature range has undergone some development since that time. Theoretical part of this bachelorś thesis describes physiology and pathophysiology of body temperature and it's regulation, the way of temperature measurement and pitfalls, that often accompanies this measurement. This thesis also reviews the historical development of therapeutic hypothermia, it's current indications and contraindications, methods of performance, describes pathophysiological processes, which are the basis of it's therapeutical effect and describes also adverse events, which accompanies this therapeutical method. The practical (research) part of bachelorś thesis is divided into two parts. The first part aims to describe the practical performance of this therapeutical method in medical intensive care unit. It aims to find out, how the temperature measurement is performed in patients after cardiac arrest in real practice, which methods are used for targeted temperature management and what is the success rate of achieving target temperature. For this analysis we choose patients after cardiac arrest, who remain unresponsive after return od spontaneous circulation , who were hospitalized in medical intensive care unit of University hospital in Pilsen from 1.1.2016 to 1.1.2017. This is a retrospective study, which in summary processes anonymous data obtained from patient medical records. This thesis in the final evaluation compares results with published data from another studies from another hospitals. This work also analyzes in detail the above mentioned patients , their initial rhythm and causes of cardiac arrest. It analyzes also mortality data after one month after cardiac arrest and neurologic outcome at discharge from hospital and compares it with literature data from another hospitals. The second part of research aims to describe subjective evaluation of targeted temperature management performance by nursing staff. This part of the research was conducted through a questionaire survey and aims to find out, how difficult is temperature management performance for nursing staff, where the nursing staff sees the main problem in using this method and, finally, analyzed what the nursing staff want to change on the application of this procedure or what they are missing.
