Abstract:

Tématem bakalářské práce je Hodnocení bolesti u pacienta na jednotce intenzívní péče. Hlavním cílem bakalářské práce, je zjistit, zda sestry hodnotí bolest správně a zda je hodnotící škála BPS objektivní a spolehlivá. Teoretická část se zabývá obecně bolestí, jak její historií, tak i rozdělením samotné bolesti na akutní, nebo chronickou formu. Nicméně teoretické část zahrnuje také hodnotící škály, které se při hodnocení bolesti dají použít, jejich původ a spolehlivost. Neopomenula jsem ani samotnou možnost léčby bolesti, jak farmakologickou, tak i nefarmakologickou. Praktickou část mé bakalářské práce, tvoří výzkumné šetření, které probíhalo na jednotce intenzivní péče v Masarykově nemocnici v Ústí nad Labem. Šetření prováděli 4 všeobecné sestry, které byli seznámeny s hodnotící škálou BPS. Bolesti se hodnotila celkem u 24 pacientů, kteří byli hospitalizování z různých příčin, například po polytraumatech, respiračním selhání, sepsi, také z důvodu kardiologického onemocnění, po abdominálních operacích, hrudních operacích a pacienti po akutním tepenném uzávěru. Všichni pacienti byli pod analgosedací a na umělé plicní ventilaci. U pacientů, kteří byli vybráni do výzkumného šetření se hodnotila bolest vždy před, během a po ošetřovatelské intervenci. Jednalo se o ošetření očí, které bylo označeno jako "nebolestivé" a endotracheální odsávání, které bylo vyhodnocení jako "invazivní ošetřovatelská intervence". Na základě výsledků výzkumného šetření, bylo zjištěno, že hodnotící škála BPS je adekvátní k hodnocení bolesti u takto závažně nemocných pacientů, kteří nejsou schopni svou bolest verbalizovat.

Abstract:

The topic of the bachelors thesis is the assessment of pain in a patient in the intensive care unit. The main goal of the bachelor thesis is to find out whether nurses evaluate pain correctly and whether the BPS rating scale is objective and reliable. The theoretical part deals with pain in general, both its history and the division od pain itself into acute or chronic form. However, the teoretical part also includes assessment scales that can by used to assess pain, their it is origin and reliability. I did not forget the high change of treating pain, both pharmacological and non-pharmacological. The practical part of my bachelors thesis consists of reseach survey, which took place in the intensive care unit at Masaryk Hospital in Ústí nad Labem. The survey was conducted by 4 general nurses who were familiar with the BPS rating scale. Pain awas assessed in a total of 24 patients who were hospitalized for various reason, such as polytrauma, respirátory failure, sespsis, also due to cardiac disease, abdominal surgery, thoracic surgery and patients after acute arterial occlusion. All patients were under analgesia and on artificial lung ventilation. In patients who were selected for the reseach servey, pain was always assessed before, during and after the nursing intervetion. These were eye treatments, which were described as " painless", and androtracheal suctioning, wich was evaluted as an "invasive nursing intervention". Based on the results of the research survey, it was found that the BPS rating scale is adequate to assess pain in such seriously ill patients who are unable to verbalize their pain.
