Application of genomics approaches in cancer diagnostics

Abstract:

Nedílnou součástí diagnostiky nádorových onemocnění je v dnešní době detekce a analýza genetických aberací v nádorové tkáni pomocí molekulárně biologickách metod, přičemž v posledních letech se stále více uplatňuje nová technologie sekvenování nukleových kyselin – tzv. sekvenování nové generace (NGS). Tato metoda umožňuje detailní vhled do genomu pacienta, a díky své univerzálnosti a snižující ceně se stává čím dál více využívanou i v klinické praxi. V kontextu nádorových onemocnění metody NGS analýzy umožňují extrémně přesnou detekci somatických mutací, které mohou pomoci dříve a precizněji diagnostikovat dané onemocnění, a tak umožnit aplikaci personalizované léčby podle individuálních potřeb pacienta na základě genetických charakteristik onemocnění. Takovýto typ léčby vede k efektivnějším terapeutickým výsledkům a potenciálně také k minimalizaci vedlejších účinků. Detekce nízkofrekventních somatických mutací zároveň umožnuje sledování klonálního vývoje onemocnění. Relativní nevýhodou, ale také vědeckou příležitostí NGS analýz je nutnost netriviálního zpracování dat, které není snadno automatizovatelné. Tato potřeba přispěla k velmi dynamickému rozvoji nového vědního oboru - bioinformatiky, který se zabývá analýzou biologických dat pomocí výpočetních metod. Bioinformatická analýza se tak stává kritickou součástí NGS experimentu, přičemž její správné nastavení a provedení vyžaduje porozumění jak biologické podstatě experimentu, tak zároveň znalost dostupných softwarových nástrojů a analytických postupů. V rámci svého doktorského studia jsem provedl bioinformatické analýzy několika typů velmi specifických projektů (12) zaměřených na detekci nízkofrekventních jednonukleotidových substitucí (SNV) a inzercí a delecí (InDel) v DNA pacientů s hematologickými malignitami. Tyto analýzy vyžadovaly komplexní optimalizace bioinformatických postupů, které se následně staly inspirací k vytvoření nástroje pro automatizované řešení tohoto problému - ToTem. Popis těchto nově navržených bioinformatických přístupů a metody na jejich automatickou optimalizaci tvoří pilíř této disertační práce. Naše studie potvrzují, že NGS může být úspěšně použito v klinickém výzkumu i diagnostice onkologických onemocnění, např. ke studiu klonální evoluce nádorů, velmi citlivé diagnostice mutací, predikci odpovědi na léčbu či ke stanovení prognózy onemocnění. Použitím programu ToTem pro optimalizaci bioinformatické analýzy bylo dosaženo několika násobně vyšší citlivosti detekce záměn na úrovni DNA oproti výchozím nastavením, což znamená zároveň navýšení potenciálu metody NGS pro tyto účely.

Abstract:

An integral part of tumor disease diagnosis is the detection and analysis of genetic aberrations in tumor tissue using molecular biology methods, whereas a new technology in nucleic acid sequencing – “Next generation sequencing” (NGS) has increasingly been used in recent years. This method offers a detailed insight into the genomic composition of the patient, and is becoming frequently used in clinical practice and probably will dominate laboratory methods in the future because of its versatility and decreasing price. In the context of tumor diseases, the NGS method allows for extremely accurate somatic mutation detection that can help to diagnose the disease faster and more precisely, thus allowing personalized treatment to be tailored to the individual needs of the patient, based on the disease’s genetic characteristics. This type of treatment leads to more effective therapeutic results and also to potentially minimize side effects. The detection of low-frequency somatic mutations also enables tracking the disease’s clonal evolution. A relative disadvantage, but also a scientific opportunity, of NGS analysis is the need for non-trivial data processing that is difficult to automate. This need has given rise to a completely new branch of bioinformatics, which deals with the analysis of biological sequences using computational methods. Bioinformatics analysis thus becomes a critical part of the NGS experiment, and its correct setting and implementation requires understanding both the biological nature of the experiment and the knowledge of available software tools and analytical procedures. In my PhD study, I have performed bioinformatics analyses of several types of very specific projects (12) focused on detecting low-frequency variants SNV and InDels from DNA of patients with hematological malignancies. These analyses required complex bioinformatics optimizations, which became the inspiration to create an automated solution for this problem - the program called ToTem. The description of these newly proposed bioinformatics approaches and the methods I have created for their automatic optimization constitute the pillar of this dissertation. The most important projects I have been involved in are described in this dissertation. Our studies confirm that NGS can be successfully used in clinical research and diagnostics on oncological diseases e.g. in studying clonal tumor development, very sensitive mutation diagnostics, predicting treatment response, or determining disease prognosis. Using ToTem to optimize bioinformatics analysis, a several times higher detection sensitivity of DNA variants was achieved than with the default settings, which thus increased the potential of the NGS method for these purposes.
