Causes of pericapillar brain haemorrhages accompanying a gunshot wounds

Abstract:

Cílem této práce bylo prostudovat soubor zemřelých v důsledku střelného poranění a pitvaných na Ústavu soudního lékařství LF MU v Brně v letech 1984 – 2007 se zaměřením na průkaz a objasnění výskytu vzdáleného poranění v podobě perikapilárního krvácení v mozku. V rámci této práce byla rovněž podrobněji statisticky zpracována některá data z pitevních protokolů z let 1998 – 2007. Za tuto dobu bylo provedeno celkem 23 502 pitev, z toho 366 pitev zemřelých v důsledku střelného poranění. Statistickým sledováním bylo zjištěno, že v této skupině zemřelých výrazně převládají muži (94%) nad ženami (6%), k úmrtí z tohoto důvodu dochází nejčastěji mezi 31 – 60 rokem věku života, výrazně převažuje poranění jednotnou střelou (79%) nad střelou mnohotnou (9%). Poranění způsobených brokovnicí bylo dokonce méně než poraněních vzniklých jateční pistolí (těch bylo 12%). Co se týče lokalizace střelného poranění na těle, absolutně převládá poranění hlavy nad jinými částmi těla. Pokud vezmeme všechna střelná poranění, tak hlava byla poraněna přímým zasažením střelou ve více než 84% případů a pokud vezmeme pouze izolované poranění hlavy bez poranění jiných částí, jednalo se o téměř 80% všech případů střelných poranění. Až daleko za poraněním hlavy je izolované poranění hrudníku (téměř 8%). Ostatní části těla (krk, břicho) byly izolovaně poraněny v minimálním množství případů, smrtelné poranění končetin dokonce nebylo vůbec zaznamenáno. U téměř 9% případů došlo k poranění více částí těla najednou buď jedinou střelou nebo mnohem častěji více střelami. Sledováním změřených hladin alkoholu bylo zjištěno, že téměř 50% zemřelých mělo negativní nález, další poměrně velká skupina zemřelých se v době smrti nacházela ve stadiu střední a těžké opilosti (dohromady cca 18%). Ze 45 874 pitev provedených na našem Ústavu soudního lékařství v letech 1984 – 2007 bylo vybráno 33 případů smrtelného střelného poranění hrudníku způsobené jednotnou střelou. Všechna úmrtí, kdy bylo při pitvě zjištěno jiné trauma, byla vyloučena ze souboru sledovaných zemřelých. V těchto pečlivě vybraných případech byla histologicky zkoumána mozková tkáň z oblasti mozkových hemisfér, bazálních ganglií, Varolova mostu, prodloužené míchy a mozečku. Mikroskopické hemoragie kolem drobných cév, především kapilár, byly nalezeny ve všech vzorcích. Tato krvácení jsou způsobena náhlými změnami krevního tlaku v důsledku účinku tlakové vlny na velké cévy hrudní při průchodu střely hrudníkem. V závěru práce je nastíněna klinická problematika těchto typů poranění, kdy je zdůrazněn zejména možný vliv na předčasný rozvoj degenerativních změn mozku.

Abstract:

The aim of this work was an examination of the file of deceased by means of gunshot wounds and whose which autopsy was performed in the Institute of Forensic Medicine Faculty of Medicine Masaryk University in Brno during years 1984 - 2007 with focus on the incidence and clarification of the presence injury in the form perivascular brain microhaemorrhages. As part of this work was also further statistical processing of some data from the autopsy protocols from years 1998 - 2007. During this period were made in total 23 502 post-mortem examinations of which 366 died as a result of the gunshot wound. It was found that in this group of deceased significantly dominated men (94%) over women (6%), to death for this reason occurs most often between 31 - 60 year age of life, greatly outweigh the single bullet injuries (79%) over the shotgun bullet (9%). Injuries caused by a shotgun were even less than the slaughter gun injuries (those 12%). As regards the localization gunshot wounds on the body, totally dominated head injury over other parts of the body. The head injury was the direct hitting bullet in more than 84% of cases, only isolated head injuries without other parts were a nearly 80% of all cases of injuries. Far behinde the head injury is an isolated injury of the chest (almost 8%). Other parts of the body (neck, abdomen) were isolated injured in the minimum number of cases, even fatal injury of the limbs was not recorded at all. Almost 9% of cases were with injuries of more parts of the body at once, namely by either a single missile or more often by multiple projectiles. By monitoring levels of alcohol was found that almost 50% of deceased had a negative finding, another relatively large group of the deceased was at the time of death at the stage of medium and heavy drunkenness (together about 18%). 33 cases of fatal penetrating chest wound by a single bullet were chosen from among 45 874 autopsies performed in our Institute of Forensic Medicine during years 1984 – 2007. All other traumatic factors were excluded, even those in the past history of the decased. In such meticulously selected cases a brain tissue was examined histologically; samples were taken from brain hemispheres, basal ganglia, the pons, the oblongate and from the cerebellum. Cufflike pattern haemorrhages around small brain vessels were found in all specimens. These haemorrhages are caused by sudden changes of the intravascular blood pressure as a result of a compression of intrathoracic great vessels by a shock wave caused by a penetrating bullet. At the end of work are outlined clinical problems of these types of injuries. It is highlighted in particular the potential impact on the early development of degenerative changes in the brain.
