Zobrazování periferních nervů pomocí pokročilých technik magnetické rezonance

Abstract:

Zobrazení magnetickou rezonancí se díky vynikajícímu kontrastu měkkých tkání stalo velmi oblíbenou zobrazovací modalitou u různých postižení PN. Vhodně doplňuje klinické a elektrofyziologické vyšetření pacienta a přináší tak další informace využitelné v rámci diagnostiky, sledování dynamiky změn v čase i terapie některých neuropatií včetně postižení struktur BP a LSP. Uplatnění tak nachází například u některých poranění PN, atypických či vzácných úžinových syndromů, tumorů PN i pooperačních kontrol. Technologický pokrok umožnil postupný rozvoj senzitivnějších a specifičtějších zobrazovacích metod MR, mezi které v rámci problematiky PN patří i takzvané pokročilé techniky - MR neurografie, DTI a MR traktografie. Tyto techniky umožňují nejen detailní strukturální 3D zobrazení struktur PN, ale i mikrostrukturální vyšetření jednotlivých svazků nervových vláken s kvantitativním hodnocením funkční integrity PN. Poskytují tak nové, pro klinickou praxi mnohdy klíčové informace o průběhu a funkci nervových svazků, jejichž získání konvenční kvalitativní metody MR neumožňují. DTI a MR traktografie PN v současném stádiu vývoje reprezentuje zejména pomocnou výzkumnou metodu. V rámci naší práce se podařilo tuto techniku zavést do klinické praxe a využít jí k vyšetření prezentované skupiny pacientů. Základna radiodiagnostiky a intervenční radiologie Institutu klinické a experimentální medicíny v Praze je tak v současné době jediným pracovištěm v ČR, kde se metoda používá. Přes některé technické limitace naše práce demonstruje možnost získat validní zobrazení PN v různých lokalitách pomocí DTI a MRT. V blízké budoucnosti, v souvislosti s vývojem hardwarových i softwarových možností MR, lze očekávat postupné rozšíření techniky DTI a MRT PN do klinické praxe i na dalších pracovištích a tím jejich širší využití. S dalším rozvojem pokročilých zobrazovacích technik a jejich integrací do rutinních vyšetřovacích protokolů MR lze v budoucnu předpokládat zkvalitnění péče o postižené pacienty, ať už formou cílené terapie bez zbytečné časové prodlevy, možností lepšího předoperačního plánování, nebo eliminací nepotřebné chirurgické intervence s pokračováním v konzervativním postupu.

Abstract:

Magnetic resonance imaging has become a popular method of peripheral nerve evaluation, due to its excellent depiction of soft tissues. This method complements the established diagnostic tools of clinical evaluation and electrophysiological studies, as it provides information enhancing diagnosis of peripheral nerve pathology. It also serves as a monitoring tool, showing changes in neuropathies over time, including brachial and lumbosacral plexus injuries. Consequently, this method is clinically significant in cases of peripheral nerve injury, atypical or rare entrapment syndromes, peripheral nerve tumors or postoperative follow-up. Technological advancement has resulted in the evolution of more sensitive and specific MR imaging techniques, which include the so called advanced techniques - MR neurography, DTI and MR tractography. These techniques allow detailed structural portrayal of peripheral nerves in 3D as well as microstructural analysis of individual nerve fascicles with quantitative assessment of their functional integrity. In conclusion, they provide new and key information on the course and function of neural fascicles, unattainable via conventional qualitative MRI techniques. DTI and MR tractography of peripheral nerves are currently, for the most part, supporting and experimental methods. In our study, we managed to implement these techniques into clinical practice, utilizing them in the diagnostic algorithm of the presented patient cohort. The Department of Radiodiagnostic and Interventional Radiology at the Institute for Clinical and Experimental Medicine in Prague is currently the only institution in the Czech Republic which implements these techniques. Although there are some technical limitations in our work, our study demonstrates the possibility of obtaining a valid representation of peripheral nerves in various regions using DTI and MRT. With evolution of MRI hardware and software techniques, we expect progressive implementation of DTI and MRT into clinical practice at other institutions, improving their availability in the near future. Progression of advanced imaging techniques and their integration into routine MR diagnostic protocols, will hopefully result in improved patient care by means of targeted therapy, faster diagnosis, improved surgical planning and the elimination of unnecessary surgical intervention in cases where conservative treatment is warranted.
