Use of the Precision Oncology Concept in the Treatment of Pediatric Patients with High-Risk Cancer

Abstract:

Precizní onkologie představuje pro dětské pacienty s vysoce rizikovým nádorovým onemocněním novodobý léčebný koncept založený na komplexní molekulárně-biologické analýze tumoru za využití moderních vysokokapacitních metod se snahou pokrýt všechny úrovně genové exprese. Výsledky získané jednotlivými analýzami pak společně vytváří rozsáhlou charakterizaci nádorové tkáně umožňující identifikaci diagnosticky, prognosticky či terapeuticky významných genových alterací ovlivňujících následnou volbu léčebné strategie a umožňujících maximální individualizaci onkologické léčby. V této diplomové práci bylo využito metod sekvenování nové generace zahrnujících celoexomové, celotranksriptomové a cílené RNA sekvenování k charakterizaci nádorové tkáně šesti pediatrických pacientů prospektivně indikovaných ke komplexnímu profilování. Dále bylo provedeno vyhodnocení kompletních výsledků molekulárních analýz u deseti retrospektivních pacientů, v rámci kterého bylo představeno široké spektrum klinicky informativních alterací.

Abstract:

Precision oncology represents a new therapeutic concept for pediatric patients with high-risk tumor that is based on a comprehensive molecular biological analysis of the tumor using modern high-throughput methods with an effort to cover all levels of gene expression. The results obtained by individual analyses then together form a comprehensive characterization of tumor tissue enabling the identification of diagnostically, prognostically or therapeutically significant gene alterations influencing the subsequent choice of treatment strategy and allowing maximal individualization of the cancer treatment. In this thesis, next-generation sequencing methods, including whole-exome, whole-transcriptome and targeted RNA sequencing, were used to characterize the tumor tissue of six pediatric patients prospectively referred for comprehensive profiling. In addition, an evaluation of the complete molecular analysis results of ten retrospective patients was performed, presenting a wide range of clinically informative alterations that can be found in a pediatric patient population.
