Evaluation of the effect of prophylactic coagulation factor concentrates in adult haemophilia patients

Abstract:

Anotace Disertační práce se zaměřuje na zhodnocení efektu profylaktické aplikace koncentrátů koagulačního faktorů u dospělých pacientů s těžkou hemofilií, zejména s ohledem na redukci počtu krvácení (celkových – ABR a kloubních – AjBR) a zohlednění spotřeby při porovnání léčby pacientů léčených on-demand a na profylaxi v období mezi lety 2013 a 2021. V rámci sledovaného období byla využita data z registru Českého národního hemofilického programu (ČNHP) v rámci celé České republiky (s výjimkou jednoho centra, které není zahrnuto v registru). Kromě celorepublikových dat se práce zaměřuje i na výsledky Centra pro komplexní péči o pacienty s hemofilií (Comprehensive Care Centre – CCC) na Oddělení klinické hematologie Fakultní nemocnice Brno. Z výsledků publikovaných dat je zřejmý výrazný pokles celkového počtu krvácení – ABR (annual bleeding rate) i kloubních AjBR (annual joint bleeding rate). I přes navýšení spotřeby při převedení pacientů z léčby on-demand na profylaxi je u hemofilických pacientů důležitá skutečnost, že při absenci kloubního krvácení nedochází k dalšímu kloubnímu poškozování a může se tak oddálit nutnost finančně náročných ortopedických výkonů.

Abstract:

Annotation The dissertation focuses on the evaluation of the effect of the prophylactic application of coagulation factor concentrates in adult patients with severe hemophilia, especially about the reduction of the number of bleeding (total and joint) and the consideration of consumption when comparing the treatment of patients treated on-demand and patients treated prophylactically in the period 2013-2021. During the monitored period, data from the register of the Czech National Hemophilia Program (CNHP) were used throughout the Czech Republic (except for one center that is not included in the register). In addition to nationwide data, the work also focuses on the results of the Comprehensive Care Center (CCC) at the Department of Clinical Hematology University Hospital Brno. The results of the published data show a significant decrease in the total number of bleedings – ABR (annual blood rate) and joint AJBR (annual joint bleeding rate). Despite the increase in consumption when patients are transferred from on-demand treatment to prophylaxis, it is important for hemophiliac joint patients that in the absence of the disease, no further joint damage occurs, thus delaying the necessity of financially demanding orthopaedic procedures.
