Quality of life of patiens with diabetic foot ulcers

Abstract:

Cíl: Cílem výzkumu bylo zjistit vliv diabetické ulcerace na kvalitu života u pacientů se syndromem diabetické nohy. Dílčím cílem bylo zhodnotit kvalitu života těchto pacientů v závislosti na pohlaví, věku, přítomnosti bolesti, délce trvání rány. Metodika: Výzkum měl kvantitativní charakter. Na sběr dat byl použitý standardizovaný dotazník Diabetic Foot Ulcer Scale (DFS). Soubor tvořilo 247 pacientů se syndromem diabetické nohy ve věkovém rozpětí 24-90 let. Sběr dat probíhal od dubna 2014 do prosince 2014 v 18 podiatrických ambulancích a ambulancích chronických ran na celém území České republiky. Výsledky: Bylo zjištěno, že nejvyšší průměrný index kvality života pacienti mají v doméně Nedodržování (84,6) a nejnižší v doméně Volný čas (49,2). Nebyl zjištěn vliv pohlaví pacientů, užívání perorálních antidiabetik a BMI na kvalitu života pacientů s diabetickou ulcerací. Vliv věku, rodinného stavu, typu diabetu, kouření a dosaženého vzdělání na kvalitu života pacientů s diabetickou ulcerací byl zjištěn vždy jen v jedné doméně. Kvalita života byla dále ovlivněna délkou trvání ulcerace a užíváním inzulínu (ve třech doménách), léčbou v ambulanci chronické bolesti a zaměstnáním pacientů (ve čtyřech doménách) a přítomností a intenzitou bolesti (přítomnost v 9 doménách a intenzita ve všech 11 doménách). Závěr: Na kvalitu života pacientů s diabetickou ulcerací má vliv bolestivost ulcerace, délka trvání ulcerace a typ zaměstnání pacientů. Vliv pohlaví ani rodinného stavu na kvalitu života pacientů s diabetickou ulcerací nebyl zjištěn.

Abstract:

Aim: To determine the effect of diabetic foot ulcers on the quality of life in patients with diabetic foot syndrome in different areas of life. A secondary aim was to assess the quality of life of these patients according to gender, age, presence of pain, duration of ulcers. Methods: The research was quantitative in nature. For data collection was used a standardized questionnaire Diabetic Foot Ulcer Scale (DFS). The sample consisted of 247 patients with diabetic foot syndrome in the age range 24-90 years. Data were collected from April 2014 to December 2014 in 18 foot clinic and outpatient chronic wounds throughout the Czech Republic. Results: The highest mean index of quality of life of patients was in domain Compliance (84.6) and lowest was in the domain Leisure time (49.2). There was not found any relationship between QoL of DFU patients and gender, the use of oral antidiabetics and BMI. There was found relationship between QoLof patients with diabetic foot ulcer and age, marital status, type of diabetes, smoking and education always in one domain. Quality of life has also relationship with the duration of ulceration and the use of insulin (in three domains), in the outpatient treatment of patients with chronic pain and employment (in four domains) and the presence and intensity of pain (presence in 9 domains and intensity in all 11 domains). Conclusion: The quality of life of patients with diabetic foot ulcers affects pain ulceration, duration of ulceration and employment type of patients. The influences of gender and marital status on the quality of life of patients with diabetic foot ulcers were not found.
