The Standard of Nursing Care - Urinary Bladder Catheterization and Permanent Urinary Catheter Care

Abstract:

Cíl: Cílem bakalářské práce bylo přepracování standardu ? Katetrizace močového měchýře a péče o permanentní močový katétr. Metodika: Návrh nového standardu byl vytvořen pro Nemocnici Podlesí, a.s. Pro sběr dat byla vybrána metoda skrytého pozorování, které bylo uskutečněno na anesteziologicko-resuscitačním oddělení této nemocnice v březnu 2014. Byly stanoveny základní okruhy sledování: hygiena rukou a dodržování platného standardu pro katetrizaci močového měchýře. Získaná data byla vyhodnocena a stala se podkladem pro vytvoření pilotního návrhu nového standardu. K jeho vytvoření byla použita šablona pro standardizované postupy Nemocnice Podlesí, a.s. Obsah standardu byl zpracován na podkladě nových poznatků v odborných publikacích a s využitím elektronických zdrojů. Pilotní návrh nového standardu byl předán k připomínkovému řízení manažerce ošetřovatelské péče. Po zpracování připomínek byla vypracována definitivní verze nového standardu, která se stala součástí standardů platných pro všechna pracoviště Nemocnice Podlesí, a.s. Výsledky: Průzkumem na vybraném oddělení byly zjištěny nedostatky jak v dodržování hygienických opatření, tak v dodržování stávajícího standardu ošetřovatelské péče pro katetrizaci močového měchýře. Rovněž byly zjištěny nesrovnalosti v nastavení stávajícího standardu, a proto byl vytvořen nový standard ošetřovatelské péče pro katetrizaci močového měchýře, který by měl snížit míru rizika nozokomiální infekce v souvislosti s tímto výkonem. Závěr: Problematika infekcí souvisejících se zdravotní péčí vyžaduje neustálou pozornost zdravotníků. Je třeba nadále vyhledávat nové poznatky a tyto pak zpracovávat pro běžnou praxi v péči o klienty zdravotnických zařízení tak, aby byla dosažena co nejvyšší míra dodržování nových postupů a tím i bezpečnosti poskytované péče.

Abstract:

Aim: The aim of the thesis was to rework and improve the standard of care for urinary bladder catheterization and the permanent urinary catheter. Methodology: A new standard of care has been designed for Podlesi Hospital. For the collection of data a method of observational research was selected, and was carried out at the high dependency unit of this hospital in March 2014. The following aspects were chosen for observation: hand hygiene and keeping the rules laid down by the existing standard of urinary bladder catheterization. The data thus obtained was evaluated and provided a basis for the pilot design of the new standard. For the creation of the design a Podlesi Hospital template for standardized processes was used. The content of the standard was supplied following the new findings in professional medical literature, using electronic media resources. The pilot project was submitted for review to the nursing care manager. The comments which she made were incorporated into the definitive version of the new standard, which has been included in the existing standards used by all departments of Podlesi Hospital. Results: The survey of the selected department found insufficiencies in both the maintenance of hygienic precautions and keeping to the existing nursing care standard of urinary bladder catheterization. In addition, there was a mismatch in the setup of the existing standard. Therefore, a new standard of care was designed, which is expected to reduce the risk of nosocomial infection related to the procedure. Conclusion: The problem of infections connected with nursing care requires continuous attention of nursing staff. It is necessary to continue searching for new facts and to include them in a day-to-day praxis of caring for clients of medical institutions in order to achieve the maximum possible level of nursing care security provision as well as to follow properly.
