Relationship between multiple sclerosis and Epstein-Barr virus infection

Abstract:

Úvod: Tato diplomová práce se zabývá autoimunitním onemocnění, které se nazývá roztroušená skleróza (dále RS) neboli Sclerosis multiplex. Projevuje se typickými zánětlivými změnami v centrálním nervovém systému, při nichž dochází k degenerativním změnám obvykle u osob ve věku mezi 16-40 rokem života. Rizikových faktorů přispívajících k této nemoci je celá řada, avšak v současné době se vědci přiklání na silnou vazbu s virem Epstein-Barrové (EBV). Tím, že tělo napadá vlastní imunitní systém, mají infekce větší šanci a mohou být spouštěčem nemocí nebo přispívají k jejich horšímu průběhu. S virem Epstein-Barrové se celosvětově během života setká do čtyřiceti let téměř 90-95 % populace. V teoretické části je popsáno samotné onemocnění roztroušenou sklerózou, její vztah s EBV a další faktory. Ve výzkumné části jsou zpracována data pacientů RS centra z Fakultní nemocnice Ostrava (FNO), u kterých se sledoval jejich klinický stav, výsledky z diagnostických metod a další faktory. Cílem výzkumné části diplomové práce bylo zjišťování možného vlivu viru Epstein- Barrové jakožto rizikového faktoru na progresi či aktivitu roztroušené sklerózy u pacientů ve FNO. Metodika: Ve výzkumné studii bylo monitorováno celkem 45 osob, z toho 32 žen a 13 mužů, kteří podstupovali léčbu v RS centru ve Fakultní nemocnici Ostrava. Podmínkou pro zařazení pacienta do studie byla přítomnost EBV protilátek a nepřítomnost progresivní formy nemoci. Tito pacienti byli sledováni po dobu 2 let od první návštěvy v RS centru. Veškerá potřebná data byla získávána z integrovaného klinického informačního systému zvaného IKIS. Analýza dat proběhla pomocí popisného statistického zpracování a k hodnocení byl použit program Stata verze 17+ včetně jeho jednotlivých statistických testů, jako je Chí-kvadrát test či Kruskalův-Wallisův test a neparametrický Wilcoxonův test (Mann-Whitney test). Výsledky: Při hodnocení průměrných hladin protilátek anti-EBV/EBNA IgG a anti- EBV/VCA IgG ve srovnání s relapsy či přítomností progrese na magnetické rezonanci nebyl prokázán statisticky významný vztah. U pacientů, kteří měli nižší hladiny vitamínu D, byl patrný stoupající trend hladin všech typů protilátek. Ovšem statisticky významný vztah se nepotvrdil ani u anti-EBV/EBNA IgG (p=0,452) a anti-EBV/VCA IgG (p=0,937). Při hodnocení rizikového faktoru kouření ve vztahu k průměrným hladinám protilátek se prokázal statisticky významný vztah mezi anti-EBV/VCA IgG (p=0,031) a kouřením. Pokud je pacient kuřák, byly u něj přítomné vyšší hladiny těchto protilátek. Závěr: Infekci virem Epstein-Barrové je stále nutné brát jako jeden z velmi významných rizikových faktorů podílejících se na onemocnění RS. Pro zvýšení klinické relevance dat je potřeba rozšířit laboratorní analýzu těchto protilátek na větší množství vzorku pacientů.

Abstract:

Introduction: This thesis deals with an autoimmune disease called multiple sclerosis (hereafter MS) or Sclerosis mutiplex. It is manifested by typical inflammatory changes in the nervous system, during which degenerative changes occur, usually in persons between the ages of 16-40. year of life. There are many risk factors contributing to this disease, but currently scientists are leaning towards a strong link with the Epstein-Barr virus (EBV). Because the body attacks its own immune system, they have a greater chance and can be the trigger of diseases or belong to their worse course. Worldwide, almost 90-95% of the population will encounter the Epstein-Barr virus during their lifetime before the age of forty. The theoretical part describes the multiple sclerosis disease itself, its relationship with EBV and other factors. In the research part, the data of patients of the RS center from the Ostrava University Hospital (UHO) are processed, in which their clinical condition, results from diagnostic methods and other factors were monitored. The aim of the research part of the thesis was to determine the possible influence of the Epstein-Barr virus as a risk factor on the progress or activity of multiple sclerosis in patients with FNO. Methodology: In the research study, a total of 45 people were monitored, of which 32 were women and 13 were men, who were undergoing treatment at the RS centre at the University Hospital Ostrava. The condition for inclusion of the patient in the study was the presence of EBV antibodies and the absence of a progressive form of the disease. These patients were followed for 2 years from the first visit to the RS centre. All necessary data were obtained from an integrated clinical information system called IKIS. Data analysis was carried out using descriptive statistical processing, and the Stata version 17+ program was used for evaluation, including its individual statistical tests such as the Chi-square test or the Kruskal-Wallis test and the non-parametric Wilcoxon test (Mann-Whitney test). Results: When evaluating the average levels of anti-EBV/EBNA IgG and anti-EBV/VCA IgG antibodies in comparison with relapses or the presence of progression on magnetic resonance imaging, no statistically significant relationship was demonstrated. In patients who had lower levels of vitamin D, an upward trend in levels of all types of antibodies was evident. However, a statistically significant relationship was not confirmed even for anti- EBV/EBNA IgG (p=0.452) and anti-EBV/VCA IgG (p=0.937). When assessing the risk factor of smoking in relation to mean antibody levels, a statistically significant relationship between anti- EBV/VCA IgG (p=0.031) and smoking was demonstrated. If the patient is a smoker, higher levels of these antibodies were present. Conclusion: Epstein-Barr virus infection should still be considered as one of the very important risk factors involved in MS. To increase the clinical relevance of the data, there is a need to expand the laboratory analysis of these antibodies to a larger sample of patients.
