Analysis of prescription of selected non-steroidal anti-inflammatory drugs in Slovakia (I. Pilot study)

Abstract:

The potential risks of non-steroidal anti-inflammatory drug treatment are still an actual problem in medical practice, as it concerns a large number of patients. These drugs are relatively cheap because most of them are accessible also generic forms. They are easily available over the counter or in higher doses by prescription, and therefore control of their consumption and use is essential due to adverse effects. Especially vulnerable in terms of drug risk are geriatric patients on long-term analgesic treatment, who often suffer from arterial hypertension and various forms of ischemic heart disease. Medical professionals such as physicians and pharmacists, to whom patients ask for help in the treatment of pain and inflammation, have a significant role in this process. This rigorous thesis also focused on the problems of non-steroidal anti-inflammatory drugs (NSAIDs). We paid attention only to those active substances such as diclofenac, ibuprofen, flurbiprofen, naproxen, nimesulide, and indomethacin, which can be recommended in the general practitioner's office and pharmacies. We were interested in their consumption in medical facilities for a specific problem of pain and at the same time the knowledge of the lay public about their use, adverse effects, duration of use, information about the medicine, and whether the given medicine solved their problem. If the use of NSAIDs is necessary, it should be always kept in mind that the risk of severe gastric bleeding increases. Patients may also be at risk of a heart attack or stroke. These risks are exacerbated if higher doses are taken, or if taken long-term. If it is necessary to use NSAIDs for longer than a week, this risk should be discussed with a doctor or pharmacist. Careful monitoring of possible adverse effects and adequate instruction on the risks of treatment must therefore form the basis of the therapeutic management of patients treated with NSAIDs. Maybe some patients would like to try other pain management treatments and so eliminate their adverse effects.

Abstract:

Potenciálne riziká antiflogistickej liečby sú stále aktuálnym problémom medicínskej praxe, nakoľko sa týka veľkej časti pacientov. Tieto lieky sú relatívne lacné, lebo pre väčšinu z nich sú prístupné aj generické formy. Sú ľahko dostupné bez lekárskeho predpisu alebo vo vyšších dávkach na lekársky predpis a preto kontrola ich spotreby a užívania je nevyhnutná vzhľadom na nežiadúce účinky. Zvlášť zraniteľní v zmysle liekového rizika sú geriatrickí pacienti na dlhodobej analgetickej liečbe, ktorí často trpia na artériovú hypertenziu a rôzne formy ischemickej choroby srdca. Zdravotnícki pracovníci, ako lekári a farmaceuti, na ktorých sa obracajú pacienti s prosbou o pomoc pri liečbe bolestí a zápalu, majú významnú úlohu v tomto procese. Táto rigorózna práca bola zameraná na problematiku nesteroidných antiflogistík. Pozornosť sme venovali len tým účinným látkam, ako je diklofenak, ibuprofén, flurbiprofén, naproxén, nimesulid a indometacín, ktoré môžu byť odporúčané v ambulancii všeobecného lekára aj v lekárňach. Zaujímala nás ich spotreba v zdravotníckych zariadeniach na konkrétny problém bolesti a zároveň vedomosti laickej verejnosti o ich užívaní, nežiadúcich účinkoch, dĺžke užívania, informovanosti o lieku a či daný liek vyriešil ich problém. Ak je užívanie NSAID nevyhnutné, treba mať stále na pamäti, že sa zvyšuje riziko vzniku závažného krvácania do žalúdka. Taktiež môže pacientom hroziť infarkt či iktus. Tieto riziká sa zhoršujú, ak sa užívajú vyššie dávky, alebo ak sa užívajú dlhodobo. Pri potrebe užívania nesteroidných antiflogistík dlhšie ako týždeň, by sa malo toto riziko prediskutovať s lekárom či lekárnikom. Starostlivý monitoring možných nežiadúcich účinkov a adekvátne poučenie o rizikách liečby musia preto tvoriť základ terapeutického manažmentu pacientov liečených nesteroidnými antiflogistikami. Možno by niektorí pacienti chceli vyskúšať aj iné možnosti liečby bolesti a eliminovať tak ich nežiadúce účinky.
