Primary examination of the patient with non-traumatic alteration of consciousness by non-medical staff

Abstract:

Tato bakalářská práce se zabývá znalostmi nelékařského zdravotnického personálu nemocniční a přednemocniční péče v diferenciální diagnostice poruch vědomí neúrazové etiologie a v algoritmu iniciálního vyšetření pacienta dle postupu ABCDE. Hlavním cílem práce bylo zjistit úroveň znalostí zdravotníků v této problematice a provést jejich porovnání mezi jednotlivými profesemi a obory. Práce je rozdělena na teoretickou a praktickou část. Teoretická část se zabývá poruchami vědomí obecně. Definuje je, popisuje patofyziologii jejich vzniku, jejich rozdělení a uvádí nejčastější neúrazové příčiny alterací vědomí. Dále přibližuje problematiku algoritmu ABCDE, rozepisuje jeho jednotlivé kroky a intervence. V praktické části jsou analyzována a interpretována data vycházející z anonymního dotazníkového šetření, jež proběhlo na vybraných odděleních jak v nemocniční, tak v přednemocniční péči. Pomocí provedené studie bylo zjištěno, že celková úroveň znalostí respondentů ve zkoumané problematice je velmi dobrá. Hodnocení výsledků dle profese ukázalo srovnatelné vědomosti u zdravotnických záchranářů a sester pro intenzivní péči. Nejmenší úspěšnost byla zaznamenána u všeobecných, praktických sester. Porovnání mezi jednotlivými obory ukázalo na lepší vědomosti u respondentů pracujících v přednemocniční péči než v péči nemocniční. Závěrem bylo respondentům doporučeno několik opatření, jimiž mohou své znalosti neustále zdokonalovat.

Abstract:

This bachelor thesis deals with the knowledge of non-medical medical staff of hospital and pre-hospital care in the differential diagnosis of disorders of consciousness of non-traumatic etiology and in the algorithm of initial examination of the patient according to the ABCDE procedure. The main goal of the work was to determine the level of knowledge of health professionals in this area and compare them between professions and disciplines. The work is divided into theoretical and practical part. The theoretical part deals with disorders of consciousness in general. It defines them, describes the pathophysiology of their origin, their distribution and lists the most common non-traumatic causes of alterations in consciousness. It also introduces the issue of the ABCDE algorithm, outlines its individual steps and interventions. The practical part analyses and interprets data based on an anonymous questionnaire survey, which took place in selected departments in both hospital and pre-hospital care. With the help of the study, it was found that the overall level of respondents' knowledge in the researched issue is very good. The evaluation of the results according to the profession showed comparable knowledge of paramedics and nurses for intensive care. The least successful was recorded for general, practical nurses. Comparisons between individual fields showed better knowledge among respondents working in pre-hospital care than in hospital care. Finally, the respondents were recommended several measures by which they can constantly improve their knowledge.
