Influence of respiratory physiotherapy on respiration parameters and postural function of patients with Parkinson´s disease

Abstract:

Tato práce se zabývá výskytem respiračních a posturálních poruch u osob s Parkinsonovou nemocí a možností jejich ovlivnění pomocí technik respirační fyzioterapie. V teoretické části je popsána charakteristika Parkinsonovi nemoci a možnosti její terapie. Dále navazují poznatky o stavu respirační a posturální funkce pacientů s tímto onemocněním. V závěru teoretické části je popsáno hodnocení poruch respirační a posturální funkce, metody funkčního vyšetření a možnosti respirační fyzioterapie. Výzkumný soubor dvaceti osob s diagnostikovanou Parkinsonovou nemocí v 2 ? 3 stupni Hohen-Yahr škály byl rozdělen do tří skupin. První skupina 9 osob absolvovala 12 týdenní skupinovou terapii zaměřenou na úpravu napřímení trupu, rozvíjení hrudníku a respirační funkce. Druhá skupina 5 osob absolvovala taktéž 12 týdenní skupinovou terapii se stejným zaměřením jako první skupina, avšak navíc obdrželi pacienti dechové pomůcky Threshold IMT a Threshold PEP pro domácí terapii. Třetí skupinu tvořil kontrolní vzorek 6 pacientů, kteří se skupinové terapie neúčastnili. Všechny tři skupiny podstoupily lékařské vyšetření neurologem, anamnestické a kineziologické vyšetření, vyšetření rozvíjení hrudníku, ústních tlaků a vyšetření posturální reaktibility. Dotazníkem UPDRS byla zhodnocena kvalita života pacientů. Bylo provedeno vstupní a výstupní měření a výsledky byly navzájem porovnány a statisticky vyhodnoceny.

Abstract:

This thesis deals with the occurance of breathing and postural disorders in people with Parkinson´s disease and with te possibilities of influenting the symptoms of disorder by techniques of respirátory physiotherapy. The paper begins with a description of the characteristics of disorder itself, including treatment possibilities. Then folows the overwiev of the respiratory and postural functions of patients with PD. The treoretical section concludes with a description of methods of respiratory physiotherapy, assesment of breathing disorders and methods of functional examination. The research group consisted of twenty people with PD at 2-3 degree Hohen-Yahr scale was divided into three groups. The first group consisted of nine people attended twelve-week group therapy focused on maintaining body posture, chest development and improving breathing functions. The second group consisted of six people attended twelve-week group therapy too. But in addition patients received respiratory aids, Treshold IMT and Treshold PEP, to home therapy. The third group consisted of six people served as a control with no therapy. Neurological examination by Neurologist, a medical history and physical examination was performed on members of all groups. The latter including examination of chest development, mouth pressures and postural reactibility. The subjects´quality of life was evaluated by questionaire UPDRS. It was performend input and output examinations and the results of measurements carried out and were compared and statistically evaluated.
