Rehabilitation protocol of a patient following traumatic detachment of the distal tendon of the humeral biceps in a clinical presentation of fibromyalgia: clinical case

Abstract:

The complete rupture of the distal tendon of the brachial biceps mainly affects male individuals, in 86% of cases it occurs on the dominant limb due to a traumatic event of the elbow and the entire structure involved. This type of trauma is initially treated with a tendon reintegration surgery in order to restore the correct functions of the limb. In cases where surgery has not been carried out, there have been deficits in muscle strength and Roma at the joint level. The patient in question is affected by fibromyalgia, defined as an autoimmune, rheumatic and somatic pathology that causes chronic widespread muscle pain and psycho-physical fatigue that can be identified by acupressure of a certain number of tender points present in various regions of the body. Consequently, it is important to observe the rehabilitation plan of the affected limb in this pathological framework. It will be assessed not only from a physical point of view, but also from a psychological one, highlighting the subject's approach to the rehabilitation project and the strategies implemented to ensure complete recovery of the joint. After the hints of anatomy, biomechanics and the pathology under examination, the methods of surgery will be assessed and a 40-day rehabilitation plan will be planned for the rehabilitation of the limb, the management of pain and finally the return to everyday life. Rehabilitation will be based on the use of manual therapy and physical therapy to treat the affected muscle and its parts. The workload will be progressive and gradual to adapt to the guidelines imposed by the doctor and the patient's pain

Abstract:

La completa rottura del tendine distale del bicipite brachiale colpisce principalmente individui di sesso maschile, nell'86% dei casi avviene a carico dell'arto dominante a causa di un evento traumatico del gomito e dell'intera struttura coinvolta. Questa tipologia di trauma viene trattata inizialmente con un intervento chirurgico di reinserimento del tendine al fine di ripristinare le corrette funzioni dell'arto. Nei casi in cui l'intervento chirurgico non ? stato praticato si sono verificati dei deficit di forza a livello muscolare e di Rom a livello articolare. Il paziente in questione ? affetto da fibromialgia, definita come una patologia autoimmune, reumatica e somatica che causa dolore cronico muscolare diffuso e stanchezza psico-fisica che pu? essere identificata mediante la digitopressione di un determinato numero di tender point presenti in varie regioni del corpo. Risulta, di conseguenza, importante osservare il piano riabilitativo dell'arto colpito in questo quadro patologico. Verr? valutato non solo dal punto di vista fisico, ma anche da quello psicologico evidenziando l'approccio del soggetto al progetto riabilitativo e le strategie attuate al fine di garantire il recupero completo dell'articolazione. Dopo i cenni di anatomia, biomeccanica e della patologia in esame, verranno valutati i metodi di intervento chirurgico e verr? programmato un piano riabilitativo di 40 giorni circa per la riabilitazione dell'arto, la gestione del dolore ed infine il ritorno alla quotidianit?. La riabilitazione si baser? sull'utilizzo di terapia manuale e terapia fisica per trattare il muscolo colpito e le sue parti. Il carico di lavoro sar? progressivo e graduale per adattarsi alle linee guida imposte dal medico ed al dolore del paziente.
