The role of serum vitamin D levels on multiple sclerosis progression

Abstract:

Úvod: Roztroušená skleróza mozkomíšní je chronické autoimunitní neurologické onemocnění, při kterém imunitní systém napadá vlastní struktury v centrální nervové soustavě. Tato neadekvátní imunitní odpověď vede k destrukci myelinových obalů nervových vláken a k postupné ztrátě axonů a oligodendrocytů. Prvotní příznaky se objevují ve vysoce produktivním věku, častěji jsou postiženy ženy. V rozvinutých zemích je roztroušená skleróza nejčastější příčinou progresivní neurologické invalidity u mladých lidí. Cílem teoretické části práce je seznámit čtenáře s problematikou roztroušené sklerózy a vitaminu D. Primárním cílem praktické části práce je vyhodnotit roli vitaminu D v progresi onemocnění RS. Metodika: Ve studii bylo sledováno po dobu 5 let celkem 106 pacientů s diagnózou CIS nebo RRRS léčených v RS centru ve Fakultní nemocnici Ostrava. Data byla sbírána z programu IKIS a následně byla podrobena analýze. V analýze byla použita základní popisná statistika, Studentův t-test a Mann-Whitney U test, testy byly hodnoceny na hladině významnosti 5 %. Soubor byl před statistickou analýzou upraven a pacienti rozděleni do skupin podle specifického bodového systému, kde nižší počet bodů značil lepší prognózu nemoci. Výsledky: Soubor pacientů byl složen ze 73 (69 %) žen a 33 (31 %) mužů. Mezi muži a ženami nebyl v průměrných hladinách vitaminu D statisticky významný rozdíl (p=0,3201). Sérové hladiny vitaminu D byly obvykle nižší u pacientů, kteří v průběhu 5 let eskalovali do vyšší linie terapie. Průměrné hladiny vitaminu D dosahující normy měly protektivní charakter vůči eskalaci, tedy tíži nemoci (OR= 0,17; IS 95 % <0,04; 0,79>). Dle bodového systému existuje signifikantní rozdíl průměrných hladin vitaminu D mezi skupinou se špatnou prognózou a skupinou s dobrou prognózou (p <0,001). Vyšší průměrné hladiny vitaminu D souvisely se stabilním stavem, ojediněle i se zlepšením, naproti tomu nízké hladiny predikovaly horší prognózu onemocnění. Závěr: Vitamin D má robustní vliv na progresi onemocnění roztroušené sklerózy i po 5 letech od počátku pozorování. Jde o velmi důležitý hormon s imunomodulačními účinky a mělo by být našim cílem dosáhnout jeho optimálních sérových hladin nejen u RS.

Abstract:

Introduction: Multiple sclerosis is a chronic autoimmune neurological disorder, in which the immune system targets one´s own structures of central nervous system. An abnormal immune response leads to demyelination of nerve fibers and subsequent damage and loss of axons and oligodendrocytes. The first symptoms usually appear in a productive age, with women being affected more frequently. Multiple sclerosis represents the most common cause of progressive neurological disability in young adults in developed countries. The aim of the theoretical part of this work is to bring a short review about multiple sclerosis and vitamin D. The primary goal of the practical part is to assess the role which vitamin D plays in the progression of multiple sclerosis. Methods: A total of 106 patients with either relapsing-remitting multiple sclerosis or clinically isolated syndrome treated in MS center in University Hospital Ostrava were observed for a period of 5 years. The data were gathered using Integrated Clinical Informative System (IKIS) and then analyzed using basic descriptive statistics, Student´s t-test and Mann-Whitney U test. The tests were evaluated at significance level of 5 %. Before starting an analysis, the patients were divided into groups according to specific point system, where less points mean better prognosis of the disease. Results: 73 (69 %) females and 33 (31 %) males were included. There was no statistically significant difference in mean vitamin D levels between males and females (p=0,3201). Serum vitamin D levels were usually lower in patients, who escalated to the second line therapy within 5 years. Normal mean serum vitamin D levels appear to have protective effect towards therapy escalation, thus the severity of MS (OR= 0.17, CI 95% <0.04; 0.79>). According to the points system, there is a significant difference between mean serum vitamin D levels between groups with poor and good prognosis (p <0.001). Higher mean vitamin D levels predicted stable disease, in some cases even improvement, while low levels predicted poor prognosis. Conclusion: Vitamin D has a massive effect on progression of multiple sclerosis even after 5 years of observation. It is a very important hormone with immunomodulatory effect and we should aim to improve its´ serum levels in general population as well.
