Objectivization of functional results after remplissage technique of shoulder joint.

Abstract:

Abstrakt v ČJ: Úvod: Glenohumerální instabilita ramenního kloubu je často spojená se vznikem kostních lézí na hlavici humeru a glenoidu. Výskyt těchto lézí vede k riziku selhání běžných artroskopických výkonů, proto se v rámci artroskopického ošetření provádí i tzv. remplissage, která má v rámci EBM studií mnohem lepší výsledky, s nižším počtem rekurentních luxací ramenního kloubu. Cíl: Cílem této diplomové práce je objektivizovat funkční výsledky vybraných svalů ramenního kloubu po operaci ramenního kloubu technikou remplissage. Metodika: Experimentální soubor tvořilo 10 probandů, kteří v minulých letech podstoupili operaci ramenního kloubu technikou remplissage. Objektivizace byla prováděna pomocí povrchové polyelektromyografie vybraných svalů, které byly snímány bilaterálně. Pro měření síly byla použita dynamometrie. Testovanými svaly byla pars acromialis deltového svalu, m. infraspinatus a m. latissimus dorsi. Následně byla hodnocena svalová aktivita a maximální svalová síla v průběhu izometrické kontrakce do vnitřní a zevní rotace. S ohledem na cíle výzkumu byly ze snímaných svalů dále zpracovány pouze získané hodnoty z m. infraspinatu. Výsledky: Výsledné hodnoty statistického zpracování neukázaly signifikantní snížení svalové síly do zevní a vnitřní rotace ani signifikantní snížení svalové aktivity m. infraspinatu při maximální izometrické kontrakci do zevní a vnitřní rotace ve srovnání operované a zdravé horní končetiny. Závěr: Technikou remplissage nedochází ke snížení svalové síly do zevní a vnitřní rotace v RAK ani ke snížení svalové aktivity m. infraspinatus při maximální volní izometrické kontrakci do zevní a vnitřní rotace, a tato technika se tedy jeví jako bezpečná z hlediska funkčních výsledků po operaci ramenního kloubu pro glenohumerální instabilitu.

Abstrakt v AJ: Introduction: The Glenohumeral instability of the shoulder joint is often associated with the formation of bone lesions on the head of the humerus and glenoid. The occurrence of these lesions results in the risk of failure of common arthroscopic procedures, therefore the so-called remplissage, which has much better results in EBM studies, with lower recurrent luxation of the shoulder joint, is performed in the arthroscopic treatment. Aims: The aim of this thesis is to objectify the functional results of selected muscles of the shoulder joint after the shoulder joint surgery using the remplissage technique. Methods: The experimental group consisted of 10 probands who underwent surgery in the past years with a remplissage technique. Objectification was performed using surface polyelectromyography of selected muscles that were scanned bilaterally. Dynamometry was used to measure force. The muscles tested were the pars acromialis of the deltoid, m. Infraspinatus and m. Latissimus dorsi. Subsequently, muscle activity and maximum muscle strength were evaluated during isometric contraction into internal and external rotation. Taking into cosideration the research objectives, only the values acquired from m. infraspinatus were further analysed. Findings: Comparing operated and healthy upper limb, the findings of statistical analysis did not show any significant reduction of muscle strength into external and internal rotation, nor significant decrease of m. Infraspinatus muscle activity at maximum isometric contraction to external and internal rotation. Conclusion: Remplissage does not reduce muscle strength to external and internal rotation in RAK, nor does it reduce m. Infraspinatus muscle activity at maximum free isometric contraction to external and internal rotation. Therefore, this technique appears safe in relation to functional results post shoulder joint surgery for glenohumeral instability.
