Quality of life monitoring in patients after surgical antireflux therapy

Abstract:

V experimentální části disertační práce byla použita nová kalibračně-měřící manometrická sonda Unisensor ECO. Na základě našich výsledků dochází k minimalizaci peroperační dislokace nové sondy a zkrácení operačního času v porovnání s klasickou perfúzní manometrií. Peroperačním měřením jednotlivých fází modelace antirefluxní manžety došlo, při porovnání obou metod, u sondy Unisensor ECO k statisticky významnému zpřesnění měření tonu LES. Na základě získaných zkušeností s peroperační manometrií jsme schopni peroperačně kontrolovat nárůst tonu LES. Předchozími výsledky s perfúzní manometrií jsme ověřili, že u pacientů s peroperačním nárůstem tonu LES do 8 mmHg dochází k minimálnímu výskytu pooperační dysfagie. V další fázi studie budeme provádět peroperační měření a modifikovat antirefluxní manžetu v případě nadměrného nárůstu tonu LES. Na základě dobrých zkušeností a výsledků jsme novou kalibračně-měřicí manometrickou sondu Unisensor ECO postoupili k patentovému řízení.

Abstract:

The goal of this prospective study was to find out whether the antireflux surgery increases QoL in our patients. We also wanted to verify whether anamnesis, functional examination of the esophagus, type of surgery and additional post-operation symptoms influence the QoL index. The goal of the experimental part was to verify the usage of a new calibration-measuring per-operation manometric probe. The Unisensor ECO per-operation manometric probe resulted in a significant reduction of the number of dislocations and an increase in the accuracy of per-operation measurement of LES tone. Patients undergoing surgical treatment for esophageal reflux disease demonstrate statistically significant improvement to their quality of life as compared with the previous phase of pharmacological therapy. Based on obtained experience with per-operation manometry, we are able to control the increase of LES tone during the operation and thereby reduce the risk of post-operation dysphagia. There was a proposal for an assignment of a patent submitted.
