Endoscopic anatomy of third ventricular floor and functional relationship. Clinicoanatomical study

Abstract:

Predložená práce vychází z oblasti stereotaxe a neuroendoskopie. Koncentruje se na zákroky na spodině III. mozkové komory, zvláště pak ve vztahu k hypothalamickým funkcím. Pro rizika spojená s operaními zákroky v oblasti hypothalamu a nástup endoskopické navigované neurochirurgie je nutné detailně prostudovat strukturu a funkci hypothalamu s implikacemi pro neuroendoskopické operace. Z hlediska rozvoje poznání struktury a funkce hypothalamu je nutné zmínit zdroje těchto poznatků, především stereotaktické neurochirurgické zákroky vyvinuté pro terapii psychiatrických poruch. Zabývali jsme se jednak změnami struktur III. komory z pohledu zobrazovacích metod (změny geometrie III. komory při hydrocefalu), dále pak možnými dopady ETV pro hydrocefalus z hlediska histologické analýzy tkáňových vzorků při zákroku odebraných a anatomickou studií struktur spodiny III. komory. Podařilo se prokázat, že korektně provedená ETV neznamená riziko přímé destrukce jaderných struktur hypothalamu. Dalším cílem práce byl souborný přehled anomálií spodiny III. komory především při hydrocefalických změnách. Studium operačních záznamů prokázalo závažné anatomické anomálie, zásadním způsobem ovlivující operační techniku. Tyto anomálie mohou postihovat každý ze tří kompartmentů spodiny III. komory - přední, střední a zadní. Vlastní studie je zaměřena především na střední část, kde se nacházejí dležité funkční struktury (corpora mamillaria, recessus infundibuli) a kde se provádí nejčastjší operace z oblasti neuroendoskopie - endoskopická třetí ventrikulostomie. Řešení představuje stereotaktická navigace endoskopického výkonu s využitím rámového nebo bezrámového systému. Změny geometrie III. komory při hydrocefalu ukazují především změny zásadního parametru pro stereotaktickou neurochirurgii - vzdálenosti CA - CP s možností ovlivnění vztahů jaderných struktur basálních ganglií ve vztahu k této linii, což je významné zjištění pro nemocné s ventrikulomegalií a plánovaným stereotaktickým funkčním zákrokem, případně před operační intervencí v hlubokých strukturách mozku, zvláště pak funkčně významných. Dalším výsledkem je stanovení intervalů normálních hodnot geometrických parametrů, vhodných pro posuzování změn rozměrů III. komory při hydrocefalu.

Abstract:

The submitted paper is based on the fundamentals of stereotactic neurosurgery and neuroendoscopy. The topic is the floor of the third ventricle, especially the relationship to hypothalamic functions. Due to the risks associated with hypothalamic surgeries and the onset of navigated neuroendoscopy it is necessary to perform meticulous study dealing with hypothalamic structures and functions and to establish consequencies for neuroendoscopic operations. Regarding the development of knowledge related to hypothalamic structure and function it is unavoidable to mention the data sources, especially stereotactic surgeries intended to treat psychiatric symptoms. The aim of the study was the analysis of changes of third ventricle based on the results of imaging techniques (geometric changes of third ventricle in hydrocephalic patients) and study of possible endoscopic third ventriculostomy consequencies studied by means of analysis of tissue samples that were taken from the ventriculostomy site and anatomical study of third ventricular floor. The result was a clear proof that correct ETV does not carry risk to the hypothalamic structures due to direct damage. The other aim of the work was to present a comprehensive analysis of anatomical anomalies affecting the floor of the third ventricle.Retrospective study of intraoperative records has proved serious anatomical anomalies, profoundly affecting surgical technique. The anomalies may affect any of the thrre compartments of the third ventricular floor anterior, middle and posterior.. Most attention is paid to the middle part, where the structures of the highest functional importance are found- mamillary bodies, infundibular recess and the most frequent neuroendoscopic surgery is performed endoscopic third ventriculostomy.New navigation systems present as a solution of these difficult and demanding tasks. The main result of the geometrical analysis of third ventricular geometry is the description of changes of the fundamental of stereotactic neurosurgery intercommisural distance AC PC, with possible changes of deep brain structures when related to the intercommisural line. This is an important findings for the patients with ventriculomegaly of any origin before planned stereotactic surgery or before any surgical intervention in the deep brain structures, especially with functional importance. Another result in the determination of geometrical parameters normal value limits, suitable for third ventricle geometry analysis and its changes in hydrocephalus.
