Nursing Process in Care of Patient with a Colostomy

Abstract:

Kolostomie je vyvedení tlustého střeva na povrch těla, ze kterého odchází stolice a plyny. Nejčastěji se zakládá na esovité kličce tlustého střeva. Tato práce je zaměřena na pacienta, u něhož byl diagnostikovaný kolorektální karcinom. Pacient byl indikován k Milesově operaci, kdy mu byla zavedena trvalá jednohlavňová sigmoideostomie a amputováno rektum. Cílem této bakalářské práce je vytvořit ošetřovatelský proces u konkrétního pacienta s nově zavedenou kolostomií, který byl hospitalizovaný na chirurgickém oddělení. Informace jsem získávala pozorováním pacienta, strukturovaným rozhovorem s pacientem, rodinou a zdravotnickým personálem a ze zdravotnické a ošetřovatelské dokumentace. Práce je rozdělena na teoretickou a praktickou část. V teoretické části práce jsou popsány jednotlivé druhy kolostomie, indikace k založení kolostomie, komplikace stomií, předoperační a pooperační péče a následný život pacienta s kolostomií. Praktická část práce je zaměřena na konkrétního pacienta s nově zavedenou kolostomií a je vytvořen plán ošetřovatelské péče. Ošetřovatelské diagnózy, které jsem stanovila, jsou zpracovány dle klasifikačního systému NANDA International Taxonomie II., včetně stanovení ošetřovatelských cílů, ošetřovatelských intervencí, realizace a hodnocení péče. U pacienta bylo stanoveno celkem 14 ošetřovatelských diagnóz, z nichž jen u jedné diagnózy nebyly požadované cíle splněny.

Abstract:

A colostomy is the discharge of the large intestine to the surface of the body surface, from which feces and gases leave. I tis most often based on the sigmoid loop of the large intestine. This thesis focused on a patient diagnosed with colorectal cancer. The patient was indicated for Miles surgery, when he underwent a permanent single-head sigmoidostomy and amputated the rectum. The aim of the Bachelor`s thesis is to creare a nursing proces for specific patient with a newly introduced colostomy who was hospitalized in the surgical department. I get information through patient observation, a structured interview with the patient, family and medical staff and from medical and nursing documentation. Thesis is split in both theoretical and practical parts. Theoretical part focuses on each type of colostomy, its identification, complications, preoperative and postoperative care and successive life of patient with colostomy. Practical part focuses on specific patient with newly aplicated colostomy, afterwise there is created plan of nursing care. The nursing diagnoses that I determined are processed according to classification system NANDA International Taxonomie II., including setting nursing goals, nursing interventions, realization and review of care. A total od 14 nursing diagnoses were determined in the patient, of which only one diagnosis did not meet the required goals.
