Postnatal quality of life of women after normal vaginal delivery and caesarean section

Abstract:

Cíl: Hlavním cílem bakalářské práce bylo zjistit a porovnat kvalitu života u žen po spontánním porodu a císařském řezu. Dalším cílem bylo srovnat kvalitu života u žen po porodu s populační normou. Metodika: Ke sběru dat byl použit dotazník, který byl rozdělen na dvě části, a to na část první - demografickou a část druhou, která se zaměřovala na kvalitu života pomocí standardizovaného dotazníku WHOQOL-BREF. Výzkum byl realizován v období od 4. 1. 2018 - 31. 3. 2018 v 5 ostravských gynekologických ambulancích, kde bylo celkem rozdáno 100 dotazníků. K vyhodnocení se vrátilo 70 dotazníků. Konečný soubor byl rozdělen na dvě skupiny dle typu porodu. Jednalo se o 46 žen, které rodily spontánně a 24 žen, které rodily císařským řezem. Výsledky: Výsledky v rámci celého souboru ukazují, že ženy po šestinedělí nejlépe hodnotí doménu č. 2 - duševní zdraví a nejhůře doménu č. 4 - životní podmínky. Když výsledky budeme rozlišovat dle typu porodu, pozitivněji hodnotily kvalitu života v rámci všech 4 domén ženy, které rodily spontánním způsobem. Několik rozdílů se ale vyskytuje. V doméně duševního zdraví byla dosažena shodná průměrná hodnota u otázky popisující negativní pocity. Ve čtvrté doméně zase ženy po císařském řezu hodnotily lépe prostředí v okolí bydliště a dostupnost zdravotní péče. Nalezené rozdíly však nebyly statisticky významné. Ovšem statisticky významné rozdíly pak byly zjištěny v kvalitě života mezi ženami po porodu a běžnou populací, kdy ženy všech věkových kategorií bez ohledu na způsob porodu hodnotily lépe doménu č. 4 - životní podmínky. Na druhou stranu ženy ve věku 31 - 40 let hodnotily signifikantně hůře doménu fyzické zdraví než běžná populace. Závěr: Těhotenství, porod i šestinedělí, je velmi významným, leč náročným obdobím v životě ženy. Zároveň je to období plné pozitivních ale i negativních změn, jak po stránce tělesné, psychické, tak i sociální. Je důležité sledovat ženy prožívající toto období, protože jsou velmi zranitelné a mnohdy i my, zdravotníci, se můžeme podílet na jejich poporodní kvalitě života. Zjištěné výsledky by mohly pomoci pochopit náročnost tohoto období a více se zaměřit na ženy po porodu a jejich potřeby. Abstract:

Goal: The main goal of this bachelor thesis was to find out and compare the quality of life in women after spontaneous childbirth and women after childbirth by caesarean section. Another goal was to compare the quality of women after childbirth with the population norm. Methodology: A questionnaire was used to collect the data and was divided into two parts, the first part - the demographic part and the second part - focused on the quality of life using the WHOQOL-BREF standardized questionnaire. The research was carried out in the period from 4 January 2018 to 31 March 2018 in 5 gynecological ambulances in Ostrava, where 100 questionnaires were distributed. 70 questionnaires were returned to the evaluation. The final set was divided into two groups according to the type of childbirth. There were 46 women who gave birth spontaneously and 24 women who gave birth by caesarean section. Results: The results within the set show that the domain 2 - mental health was evaluated by women after the puerperium as the best and the domain 4 - living conditions as the worst. By distinguishing the results according to the type of childbirth, the quality of life within all 4 domains were positively evaluated by women who gave birth spontaneously. However, there are a few differences. In the domain of mental health, a question describing negative feelings has reached the same average value. In the domain 4, women after a caesarean childbirth evaluated the neighbourhood and availability of health care better. The differences that were found were not statistically significant. However, the statistically significant differences were found in quality of life between women after childbirth and a normal population, where women of all ages evaluated the domain 4 - living conditions better. On the other hand, women aged 31-40 evaluated the domain of physical health significantly worse than the normal population. These results were statistically significant. Conclusion: Pregnancy, childbirth and puerperium is a very important but difficult period in a woman's life. At the same time, it is a period full of positive and negative changes -physical, mental and social. It is important to monitor women experiencing this period because they are very vulnerable and often we, health professionals, can participate in their puerperal quality of life. The findings could help understand the complexity of this period and focus more on women after childbirth and on their needs.
