Hodnocení omezení běžných denních aktivit u pacientů s vertebrogenním onemocněním

Evaluation of limitations in activities of daily living in patients with vertebrogenic disease

Abstract:

Cíl: Cílem diplomové práce bylo zjistit míru omezení běžných denních aktivit v důsledku bolesti zad (bederní páteře) a oblasti, ve které jsou pacienti nejvíce omezeni. Metodika: Výzkum má charakter kvantitativního šetření. Ke sběru dat, byl použit standardizovaný dotazník Oswestry disability index ODI. Pro měření intenzity bolesti byla použita hodnotící škála VAS. Pacienti vyplňovali dotazníky dvakrát, před zahájením a po ukončení fyzioterapeutické léčby, která obvykle trvá 5-6 týdnů. Výzkumný soubor tvořilo 211 respondentů. Výsledky: Výzkumným šetřením byla zjištěna míra omezení celého souboru 34,8 % což je střední disabilita. S věkem se průměrná hodnota ODI zvyšovala. Nejvyšší hodnota omezení běžných denních aktivit (45 %) byla ve skupině 71 let a více což odpovídá těžké disabilitě. Dle věku mimo oblast 8 sexuální život (p = 0,0837) byl zjištěn statistický významný vztah ve všech oblastech ODI. Zvedání, společenský život a osobní péče patřily k nejvíce omezujícím oblastem v rámci funkčního ome?zení. Výsledky výzkumu rovněž prokázaly vysokou korelaci mezi ODI a VAS (p = 0,6907). Mezi muži a ženami nebyl zjištěn statisticky významný rozdíl v hodnocení jednotlivých oblastí ODI (p = 0,1495). Výsledky prokázaly statisticky významný rozdíl ODI před a po ukončení fyzioterapeutické léčby (p < 0,001). Závěr: Z výsledků výzkumu vyplývá, že se funkční omezení ODI zvyšuje s věkem. Po absolvování fyzioterapeutické léčby došlo k výraznému snížení intensity bolesti a funkčního omezení ODI.

Abstract:

Aim: The aim of the thesis was to determine the limit normal daily activities due to back pain (lumbar spine) and the areas in which patients are most limited. Methods: Quantitative research methods were used. Data were collected by stan-dardized questionnaire Oswestry disability index ODI. The intensity of low back pain was evaluated by using a scoring system VAS. The respondents were asked to fill the questionnaires twice, before and after completion of physiotherapy treat-ments, which usually lasts 5-6 weeks. The research sample consisted of 211 respon-dents. Results: The mean overall ODI score of all respondents was 34.8 % which is the average disability. The average value of ODI increased with age. The highest value for limiting activities of daily living (45 %), were in the group 71 years and over which corresponds to severe disability. According to age, except of the area 8 sex life (p = 0,0837), were found out a statistically significant relationships in all areas ODI. Lifting, social life and personal care were the most limited areas of the functional limitation. The correlation coefficient between VAS and the mean ODI score showed highly positive correlation (p = 0,6907). However there were no significant correlations between men and women (p = 0,1495). Our study showed a sta?tistically significant difference in ODI before and after physiotherapy treatment (p < 0,001). Conclusion: The research results show that the disability ODI score increases with age. After physiotherapy treatment, we noticed a significant decrease in pain intensity and improvement in daily functions.
