Laboratory examination in Borreliosis

Abstract:

Tato diplomová práce podává stručný teoretický přehled o lymeské borrelióze a jejím původci (Borrelia burgdorferi). Věnuje se jednotlivým klinickým formám a jejich diagnostice. Praktická část se podrobněji zabývá diagnostickým procesem u neuroborreliózy. V práci je zdůrazněna problematika interpretace výsledků vyšetření. Cíl práce: Podat ucelený pohled na soubor pacientů vyšetřených pro suspektní neuroborreliózu (n = 147). Zhodnocení významu vyšetřených parametrů, konfrontace výsledků s literárními údaji. Metody: Retrospektivní analýza vyšetření provedených v období 1.1.2004 ? 31.12. 2007 v laboratoři Ústavu klinické biochemie Fakultní nemocnice Ostrava a na Zdravotním ústavu se sídlem v Ostravě. Závěr: U většiny pacientů s pravděpodobnou neuroborreliózou nelze předpokládat pozitivitu všech parametrů uváděných v literatuře. Výsledky laboratorních testů je třeba hodnotit v kontextu se všemi dostupnými informacemi (anamnéza, klinický obraz apod.). Konečné hodnocení má pravděpodobnostní charakter. V současnosti není k dispozici žádná diagnostická metoda, jejíž výsledek by sám o sobě potvrdil diagnózu.

Abstract:

This thesis makes a theoretical survey of lyme borreliosis and the causative agent (Borrelia burgdorferi). It is devoted to the particular clinical forms and their diagnostics. The second part focuses on diagnostic process of neuroborreliosis. There is an emphasis on interpretation of investigation results in this thesis. Objective: To give the insight into a group of patients who were suspected of suffering from neuroborreliosis (n = 147). To value the importance of the examined parameters and to confront the results with literary sources. Methods: The retrospective analysis of the investigation results done in the period of 1.1.2004 ? 31.12.2007 in laboratory of Ústav klinické biochemie Fakultní nemocnice Ostrava and in Zdravotní ústav se sídlem v Ostravě. Conclusion: The parameter values given in literature which show evidence of neuroborreliosis cannot be applied to all the cases. It is necessary to interpret the results of laboratory tests while having all the available information (anamnesis, clinical manifestation etc.). We have to consider that the final resolution has a kind of uncertainty, because recently there is no diagnostic method whose result would prove the diagnosis on its own.
