Case Management in a Multidisciplinary Palliative Care Team

Abstract:

Diplomová práce je zaměřena na case management v multidisciplinárním paliativním týmu a jeho nástroje, konkrétně odborné sociální poradenství. Na začátku teoretické části jsou popsána teoretická, empirická a metodologická východiska práce. Cílem teoretické části je popsat case management v multidisciplinárním paliativním týmu a jeho jednotlivé nástroje, konkrétně význam odborného sociálního poradenství. Cílem výzkumné části bylo zjistit, jak vnímají osoby na pozici koordinátora v multidisciplinárním paliativním týmu (tj. sestry, zdravotně-sociální pracovníci, sociální pracovníci aj.) case management - konkrétně význam odborného sociálního poradenství jako jednoho z jeho nástrojů. Při výzkumu byla využita kvalitativní výzkumná strategie. Polostrukturované rozhovory s komunikačními partnery byly transkribovány a zpracovány metodou tematické analýzy. Byly interpretovány a zhodnoceny výsledky výzkumu.

Abstract:

The thesis focuses on case management in a multidisciplinary palliative care team and its tools, namely professional social counselling. At the beginning of the theoretical part, the theoretical, empirical and methodological background of the thesis is described. The aim of the theoretical part is to describe case management in a multidisciplinary palliative team and its different tools, specifically the importance of professional social counselling. The aim of the research part was to find out how persons in the position of coordinator in a multidisciplinary palliative team (i.e. nurses, health-social workers, social workers, etc.) perceive case management - specifically the importance of professional social counselling as one of its tools. A qualitative research strategy was used in the research. Semi-structured interviews with communication partners were transcribed and processed using the thematic analysis method. The research findings were interpreted and evaluated.
