Farmakoekonomická analýza nákladů na hospitalizaci pacientů s akutním srdečním selháním

Pharmacoeconomic analysis of in-hospital costs in acute heart failure patients

Abstract:

Akutní srdeční selhání (ASS) je klinický syndrom se špatnou prognózou. Jeho incidence i prevalence má neustále vzrůstající tendenci, což vede ke zvyšování výdajů ve zdravotnictví. Analýzou přímých hospitalizačních nákladů z pohledu plátce zdravotní péče byly zjištěny průměrné náklady na hospitalizaci pacienta s ASS ve výši 92515 Kč za 7 dní (medián). Spotřeba zdrojů se liší v závislosti na klinické charakteristice pacienta. Signifikantně vyšší náklady byly zaznamenány u pacientů s ASS nově vzniklým než u pacientů s akutní dekompenzací chronického srdečního selhání (průměrně 101139 Kč vs. 79154 Kč). Nejčastěji zaznamenanou vyvolávající příčinou hospitalizace byly akutní koronární syndromy (44,5 % pacientů) spojené s celkově nejvyššími průměrnými náklady na hospitalizaci (121797 Kč). Variabilita ve struktuře celkových nákladů mezi jednotlivými pacienty byla ovlivněna prováděnými nákladnými výkony angiologickými a arytmologickými. Cena za pobyt na lůžku je ve srovnání se zahraničím velice nízká, bez ohledu na typ selhání či etiologii tvoří okolo 30 % celkových nákladů. Závažnost stavu pacientů dokládala vysoká potřeba pobytu na JIP (67,4 % pacientů) o délce 3 dny. Kardiochirurgická léčba (CABG u 5 % pacientů s ASS) byla spojena s velmi vysokými náklady (průměrně 310781 Kč). Hodnocením rehospitalizací pacientů byl zjištěn nárůst průměrných nákladů ve skupině se dvěma hospitalizacemi o 18 %. Výsledky předložené disertační práce poskytly detailní přehled o nákladech na léčbu ASS v jednom z kardiologických center v České republice. Díky nim se akutní kardiologie stává jednou z nejlépe farmakoekonomicky prozkoumaných oblastí u nás.

Abstract:

Acute heart failure (AHF) is clinical syndrome associated with a poor prognosis. Both prevalence and incidence increase which means the growth in healthcare systems expenditures. The analysis of direct medical costs from the perspective of the health care payer assessed in AHF patients the mean in-hospital cost CZK 92,515 (€ 3,364) during median length of stay 7 days. The diversity of resource use was found in AHF patients according to clinical characteristics. Significantly higher costs were found in patients with new-onset AHF (mean CZK 101,139; € 3,678) than in patients with acute decompensation of chronic heart failure (CZK 79,154; € 2,878). Acute coronary syndrome was the major eliciting factor for hospitalization due to AHF (present in 44.5 % of patients) and was the most expensive (CZK 121,797; € 4,429) in comparison with all other precipitating factors. The heterogeneity of resource use was largely a reflection of interventions undertaken (particularly if revascularisation or anti-arrhythmic therapy was provided). The room rate formed around 30 % of the total cost in all AHF patients which is very low compared to other studies from abroad. The severity of disease was illustrated by the high need of intensive care unit stay (67.4 % of patients) with median length of stay 3 days. The cardio-surgery (CABG in 5 % of AHF patients) was concerned with very high cost (mean CZK 310,781; € 11,301). The increase in mean in-patient cost between the first and second hospitalization was 18 %. The Ph.D. thesis provides the overview of resource use during the management of acute heart failure in renowned cardiology centre in the Czech Republic. Acute cardiology became the best explored field in pharmacoeconomic research in our country.
