The changes of woman´s psyche during the puerperium

Abstract:

Cíl: Cílem práce bylo zjistit výskyt psychických změn u žen v ranném šestinedělí a zhodnotit tyto změny pomocí Edinburghské škály postnatální deprese. Metodika: Průzkumný soubor tvořilo 100 žen hospitalizovaných ve Fakultní nemocnici v Ostravě, Vítkovické nemocnici a Městské nemocnici v Ostravě. Dotazníkové šetření probíhalo v období 1. 11. 2012 až 31. 1. 2013 a bylo realizováno standardizovaným dotazníkem Edinburghské škály postnatální deprese. Výsledky: Průzkumným šetřením bylo zjištěno, že 25 žen ze souboru popisuje svůj emocionální stav tak, že podle Edinburghská škály postnatální deprese jsou zařazeny do skupiny rizikových pro vznik poporodní deprese. Vysoké riziko pro vznik poporodní deprese měly 4 žen. Zbylých 71 žen hranici rizikovosti nepřesáhlo, ale dle jejich bodového hodnocení i u nich proběhly určité emoční změny. Pouze 4 ženy měly skóre 0 bodů, dá se tedy říci, že u nich žádné emoční změny neproběhly. Dále se zjistilo, že podle průměrů bodů mají větší sklon k poporodní depresi ženy ve věku 15?25 let, prvorodičky s nižším vzděláním. Závěr: Psychiku ženy v poporodním období ovlivňuje celá řada faktorů a žen s rizikem poporodní deprese je nezanedbatelné procento. Součástí zdravotní péče o ženu v průběhu hospitalizace na oddělení šestinedělí by mělo být vyhledávání a sledování žen s rizikem vzniku poporodní deprese. Proto je důležité, aby měl zdravotnický personál znalosti z oboru psychologie a psychiatrie a uměl rozlišit v poporodním období běžnou emoční reakci od závažné psychické poruchy.

Abstract:

Objective: The aim of my thesis was to find out occurrence of women?s psychical changes during early puerperium and evaluate them according to The Edinburgh Postnatal Depression Scale. Methodology: The pilot group included 100 women hospitalized in the Faculty Hospital in Ostrava, the Hospital in Vítkovice and the Municipal Hospital in Ostrava. The questionnaire survey was carried out from 1 November 2012 to 31 January 2013 according to the Edinburgh Postnatal Depression Scale standardized questionnaire. Results: By the questionnaire survey it was found out that 25 women from the pilot group fell into the high-risk group as for the appearance of post-natal depression according to the Edinburgh Postnatal Depression Scale. Four women were at a high risk of appearance of post-natal depression. The remaining 71 women did not exceed the risk level but on the basis of their point evaluation certain emotional changes occurred as well. Only 4 women had 0 points score so it can be said that no emotional changes occurred. Furthermore, it was found out that according to the average number of points women between the age of 15?25 and women with a lower level of education who give birth for the fist time had a bigger tendency to post-natal depression. Conclusion: The woman?s psyche is influenced by many factors during the post-natal period and there is a significant percentage of women at the risk of post-natal depression. The women?s health care during hospitalization at the department of puerperium should include searching and monitoring women with a risk of appearance of post-natal depression. Therefore it is essential for the medical staff to have knowledge of psychology and psychiatry and to be able to differentiate a common emotional reaction from a serious psychical disorder during the post-natal period.
