Genetické polymorfismy a dynamika vybraných aktivačních markerů u dětských pacientů se septickými stavy

Genetic polymorphisms and the dynamics of selected markers of activation in children with septic conditions

Abstract:

Sepse patří mezi nejvíce ohrožující stavy na jednotkách intenzivní péče pro děti i dospělé. I přes značné pokroky ve výzkumu stále nemáme k dispozici ucelenou teorii komplexně popisující patogenetické procesy v sepsi. Značná variabilita v průběhu septických stavů může být kromě jiného způsobena variantami genů kódujících jednotlivé složky imunitního systému. Dosud existují pouze omezená data popisující vliv jednonukleotidových polymorfismů a jejich kombinací na riziko rozvoje a tíže průběhu septických stavů u dětské populace. Prvním cílem studie bylo stanovení rizikovosti kombinací jednonukleotidových polymorfismů v genech kódujících klíčové molekuly účastnící se zánětlivé odpovědi organismu. Druhým cílem bylo studium dynamiky vybraných aktivačních markerů v závislosti na rizikovosti kombinací studovaných polymorfismů, což by v důsledku mohlo vést k lepšímu pochopení patofyziologických procesů v sepsi, a tím i nepřímo ke kvalitnější péči o pacienty.Ve studii byly hodnoceny jednonukleotidové polymorfismy pětice genů, konkrétně baktericidního a permeabilitu zvyšujícího proteinu (BPI, rs5743507), lipopolysacharid vázajícího proteinu (LBP, rs2232618), toll-like receptoru 4 (TLR 4, rs4986790), proteinu tepelného šoku 70 (HSP 70, rs2227956) a interleukinu 6 (IL-6, rs1800795) u 598 dětských pacientů přijatých na jednotku intenzivní péče s horečkou, syndromem systémové zánětlivé odpovědi, sepsí, těžkou sepsí, septickým šokem a syndromem multiorgánové dysfunkce. Jako kontrolní skupina bylo zahrnuto 529 zdravých jedinců. Byly zkonstruovány vícerozměrné kontingenční tabulky, které byly statisticky vyhodnoceny pomocí log-lineárních modelů. U vybraných pacientů byla pomocí ANOVA modelu hodnocena dynamika interleukinu 6, C-reaktivního proteinu a prokalcitoninu ve vztahu k demografickým parametrům, vybraným genetickým polymorfismům a jejich kombinacím. Detailní analýzy odhalily statisticky signifikantní rozdíly ve struktuře kombinací jednonukleotidových polymorfismů mezi skupinou pacientů a kontrolní skupinou, stejně jako mezi skupinou pacientů s nejtěžšími stavy (těžká sepse, septický šok a syndrom multiorgánové dysfunkce) a kontrolní skupinou. Tento přístup umožnil popsat nízce, středně a vysoce rizikové kombinace genetických polymorfismů v rámci rozvoje a následné tíže septických stavů. U podskupiny 28 pacientů s těžkou sepsí a septickým šokem byly nalezeny dílčí, statisticky signifikantní rozdíly v dynamice aktivačních markerů v závislosti na rizikovosti specifických kombinací jednonukleotidových polymorfismů. Získané poznatky by mohly být využity v rámci predikce rozvoje a tíže septických stavů, zejména pak v kombinaci s dalšími proměnnými v rámci konceptu PIRO.

Abstract:

Sepsis is among the most threatening conditions in intensive care units for children and adults. Despite significant advances in research, we still lack a coherent theory describing the complex processes in the pathogenesis of sepsis. Considerable variability in the course of sepsis can be caused, among other things, by variants of the genes encoding the individual components of the immune system. Currently, there are only limited data available for the description of the influence of single nucleotide polymorphisms and their combinations on the risk of the development and severity of the course of sepsis in the paediatric population. The first objective of the study was to determine the risk of specific combinations of single nucleotide polymorphisms in the genes encoding the key molecules involved in the inflammatory response. The second objective was to study the dynamics of the selected markers of activation and its dependence on the risk combinations of the polymorphisms under study. This could consequently lead to a better understanding of the pathophysiological processes in sepsis and thus, indirectly, to better patient care. The role of single nucleotide polymorphisms was evaluated for five genes, namely for bactericidal permeability increasing protein (BPI; rs5743507), lipopolysaccharide-binding protein (LBP; rs2232618), toll-like receptor 4 (TLR4; rs4986790), heat shock protein 70 (HSP 70; rs2227956), and interleukin 6 (IL-6; rs1800795) in 598 children aged 0 to 19 years admitted to a paediatric intensive care unit with fever, systemic inflammatory response syndrome, sepsis, severe sepsis, septic shock, or multiple organ dysfunction syndrome. A control group of 529 healthy individuals was included. Multi-way contingency tables were constructed and statistically evaluated using log-linear models. In selected patients, the ANOVA model was used to evaluate the dynamics of interleukin 6, C-reactive protein and procalcitonin in relation to the severity of sepsis, selected genetic polymorphisms and their combinations. Detailed analyses revealed statistically significant differences in the structure of the combinations of single nucleotide polymorphisms between patients and the control group, as well as between patients with the most severe conditions (severe sepsis, septic shock and multiorgan dysfunction syndrome) and the control group. This approach allowed for the description of the low-, medium- and high-risk combinations of genetic polymorphisms for the development and subsequent severity of sepsis. In the subgroup of 28 patients with severe sepsis and septic shock, partial, statistically significant differences in the dynamics of activation markers dependent on the risk of specific combinations of single nucleotide polymorphisms were found. The findings presented here could be used in the context of predicting the development and severity of sepsis, especially in combination with other variables within the PIRO concept.
