Problems of anterior cruciate ligament injuries in Football athletes

Abstract:

ABSTRAKT Úvod: Ve své bakalářské práci jsem se zabývala problematikou předního zkříženého vazu kolenního kloubu u sportovců, konkrétně fotbalistů. Cíl práce: Cílem bakalářské práce je přiblížení problematiky předního zkříženého vazu, možnosti léčby, volby fyzioterapeutických konceptů a postupů po plastice LCA kolenního kloubu. Metodika: V mé bakalářské práci je zpracována kazuistika 41 letého pacienta po plastice předního zkříženého vazu pravého kolenního kloubu. Vstupní vyšetření, které jsem provedla, zahrnovalo měření antropometrie a goniometrie, svalový test, vyšetření zkrácených svalových struktur a specifické testy stability kolenního kloubu. Pro pacienta byl navržen krátkodobý fyzioterapeutický plán, který se skládal z 10 individuálních terapií. V první části jsem se zaměřila na obnovení rozsahu pohybu, zmírnění otoku, zlepšení svalové síly a na práci s jizvou a jejím okolím. Využívala jsem cvičení v uzavřených a otevřených kinematických řetězcích, metodu proprioceptivní neuromuskulární facilitace (PNF), stabilizaci trupu podle postupu dynamické neuromuskulární stabilizace (DNS), pomůcky jako theraband a overball. Kontrolní vyšetření jsem provedla po 5. a 10. terapii. Po kontrolním vyšetření jsme se dále zaobírali zvětšením rozsahu pohybu kolenního kloubu, zvětšení svalové síly a zkrácenými strukturami. V průběhu poslední terapie byl proveden výstupní kineziologický rozbor, na jehož základě jsem navrhla dlouhodobý plán fyzioterapie. Výsledky: Došlo ke zmírnění otoku pravého kolenního kloubu, zvětšení rozsahu pohybu pravého kolenního kloubu a k normalizaci svalové síly v oblasti kolenního kloubu. Závěry: Pozitivní výsledek terapie, díky němuž bylo dosaženo téměř plného rozsahu pohybu, zmírnění otoku a zvětšení svalové síly, přisuzuji především dobré spolupráci s pacientem. Docházel poctivě na veškeré terapie a mimo společná sezení se věnoval individuálně zadaným autoterapiím.

Abstract:

Introduction: The bachelor's thesis deals with the issue of the anterior cruciate ligament (ACL) of the knee joint in athletes, specifically footballers. The objective: The aim of the bachelor's thesis is to approach the issue of the ACL treatment options, choice of physiotherapeutic concepts and procedures after plastic surgery of the ruptured ACL of the knee joint. Methodology: In the bachelor's thesis, a case report of a patient after ACL surgery of the right knee joint is processed. The initial examination included anthropometry and goniometry measurements, muscle test, and specific knee stability tests. A short-term physiotherapy plan was designed for the patient, which consisted of 10 individual therapies. The first part focused on restoring the range of motion, reducing swelling, improving muscle strength and working with the scar and its surroundings. Exercise in closed and open kinematic chains, Proprioceptive Neuromuscular Facilitation (PNF), and dynamic neuromuscular stabilization method for torso stabilization were applied and aids such as TheraBand and over ball were used. Follow-up examination was performed after the 5th and 10th therapy. Results: The swelling of the right knee joint reduced, the range of motion of the right knee joint increased, and the muscle strength in the area of the knee joint normalised. Conclusions: The positive result of the therapy, thanks to which almost the full range of motion, reduction of swelling and increase of muscle strength were achieved, attribute mainly to good cooperation with the patient. The patient attended all therapies regularly and, apart from common sessions, he performed all assigned autotherapies responsibly.
