Long-term outcome of the operative treatment of the degenerative lumbar spine disorder

Abstract:

Teoretická a vlastní klinická část práce se zaměřuje na časté téma hodnocení výsledků operační léčby degenerativního onemocnění bederní páteře. K danému úkolu přistupuje z pohledu hodnocení kvality vlastního života pacientů, kteří podstoupili operační zákrok. Sleduje tedy v souhrnu všechny nejčastější diagnózy (prostá hernie disku, spinální stenóza, osteochondróza či osteoartróza), které se skrývají pod pojmem degenerativního onemocnění bederní páteře. Hlavním cílem je prokázat zlepšení kvality života pacientů v závislosti na provedeném zákroku z dlouhodobého pohledu. Ke sledování kvality života je použito dotazníkových metod. Toto sleduje u více jak 180 pacientů Ortopedické kliniky FN Brno, kteří podstoupili operační zákrok na bederní páteři. V části experimentální se pak zaměřuje na velmi aktuální téma etiologie tohoto onemocnění z pohledu genetiky. Sleduje možnou závislost jednonukleotidového polymorfizmu (SNP) genů pro kolagen IX a matrixmetaloproteinázy 9 (MMP9) u pacientů s hernií disku versus výskytu tohoto polymorfizmu u zdravé kontrolní skupiny osob české populace. Dále sleduje i zastoupení alely HLA-B27 u obou skupin osob a snaží se nalézt možnou příčinnou souvislost mezi degenerativním onemocněním bederní páteře a přítomností této alely. Vlastními výsledky z daných dotazníků vyplývá zlepšení kvality života pacientů po operaci bederní páteře, které se udržuje i po více letech od operace. Na vlastní indikaci operace se však musí podílet mezioborová spolupráce ortopeda, neurologa i psychologa. Operační léčba degenerativního onemocnění páteře má své nezastupitelné místo v paletě léčby tohoto onemocnění. Po stránce genetické analýzy se nám však nepodařilo prokázat statisticky signifikantní výskyt závislost jenonukleotidových polymorfizmů studovaných genů a degenerativního onemocnění bederní páteře, i když existuje jistá marginální spojitost. Toto téma by si jistě zasloužilo další práci, jelikož se vzrůstajícím počtem osob v obou sledovaných skupinách můžeme v budoucnu tento vztah nalézt.

Abstract:

The theoretical and clinical part of the thesis is focused on the frequent topic of evaluation of operative treatment of lumbar spine degenerative disease. The thesis approaches to a given task from the perspective of evaluation of quality of life of patients who underwent surgery. The thesis monitors in total all the most frequent diagnosis (simple hernia disk, spinal stenosis, osteoarthritis or osteochondrosis), which are hidden under the term degenerative lumbar spine disease. The main objective is to demonstrate the quality of life of patients depending on the performed medical intervention from the long term perspective. To monitor the quality of life questionnaire method is used. This means monitoring of more than 180 patients of The Orthopaedic Department of The University Hospital Brno, who underwent surgery on the lumbar spine. Then in the experimental section it focuses on a very hot topic of etiology of this disease from the perspective of genetics. It monitors potential dependence of a single nucleotid polymorphism (SNP) of genes for collagen IX and matrixmetaloproteinasis 9 (MMP9) in patients with disc herniation versus occurrence of this polymorphism in healthy control groups of the Czech population. It also monitors the representation of allele HLA-B27 in both groups of people trying to find a causal link between the degenerative lumbar spine disease and presence of this allele. The own results of the questionnaires show the improvement of the quality of life of patients after lumbar spine surgery, which persists even several years after the operation. Interdisciplinary collaboration of orthopedist, neurologist and psychologist must however participate in the own operation indication. Surgical treatment of degenerative spine disease plays an irreplaceable role in a variety of treatment of this disease. However from the genetic analysis point of wiew we failed to prove a statistically significant incidence of addiction of the single nucleotid polymorphisms of studied genes and the degenerative disease of the lumbar spine, although there is some marginal connection. This topic would certainly deserve further work, because while the number of people increases we may find in the future this relationship in both treatment groups.
