Cílená terapie B-lymfoidních malignit: úloha molekuly CD20 a možnosti optimalizace anti-CD20 imunoterapie

Targeted therapy of B-lymphoid malignancies: the role of CD20 molecule and the possibilities of optimizing anti-CD20 immunotherapy

Abstract:

Přestože je molekula CD20 využívána jako primární cíl v imunoterapii B-lymfoidních malignit již více než 20 let, stále není zcela vysvětlen způsob její regulace a funkce. Pro objasnění funkce molekuly CD20 byly proto v této práci technologií CRISPR/Cas9 vygenerovány CD20 knockout buněčné linie reprezentující různé B-lymfoidní malignity a změny v aktivitě a funkčních vlastnostech těchto buněk byly srovnány s jejich WT protějšky pomocí různých buněčných esejí. I když je aplikace anti-CD20 imunoterapie (např. rituximabu) v klinice ze začátku velmi úspěšná, opakované cykly léčby často vyústí ve ztrátu exprese molekuly CD20 na povrchu maligních B lymfocytů a následně v rezistenci k léčbě. Možnost terapeutické modulace exprese CD20 by tak mohla vést ke zvýšení efektivity anti-CD20 imunoterapie. Byl proto proveden screening různých epigenetických inhibitorů na buněčné linii rezistentní k rituximabu se sníženou expresí CD20 s cílem najít epigenetické enzymy, které jsou schopny zvýšit expresi molekuly CD20 a senzitivitu k rituximabu. Zároveň byl využit celogenomový CRISPR/Cas9 knockout screening v kombinaci s aplikací rituximabu k identifikaci genů, které hrají zásadní roli v odpovědi na rituximab u maligních B lymfocytů. Kombinací těchto 2 různých screeningových metod byl popsán možný mechanismus modulace exprese molekuly CD20, který má potenciál být využit pro zvýšení efektivity anti-CD20 imunoterapie.

Abstract:

Despite the fact that the CD20 molecule is used as a prime target in the immunotherapy of B-lymphoid malignancies for over 20 years, its exact regulation and function remain unknown. To elucidate CD20 function, CD20 knockout B-cell lines were generated in this thesis with the use of CRISPR/Cas9 technology, and their activity and functional responses were compared to wild-type cells in multiple cellular assays. Although effective at first, repeated cycles of anti-CD20 treatment (e.g. rituximab) in the clinic often result in the loss of CD20 expression from the surface of malignant B cells and consequently in therapy resistance. Therefore, the possibility to modulate CD20 levels seems an appealing strategy to enhance the efficacy of anti-CD20 immunotherapy. An epigenetic drug screen was performed on a rituximab-resistant cell line with low CD20 expression to identify epigenetic enzymes that could enhance CD20 expression and sensitivity to rituximab. Also, the whole-genome CRISPR/Cas9 knockout screen combined with rituximab treatment was used to pinpoint genes that play a role in response to rituximab treatment. By combining the results of these two different screening methods, novel possible mechanism of CD20 expression modulation was revealed, which could be used to enhance the anti-CD20 immunotherapy efficiency.
