Dynamic and static-progressive orthoses in complex hand injuries.

Abstract:

TITLE OF THE THESIS: DYNAMIC AND STATIC-PROGRESSIVE SPLINTING IN COMPLEX HAND INJURIES. (An experimental research thesis) The purpose of this thesis was the study of the best rehabilitative methods in the presence of complex hand injuries. In such events, multi-tissue damage (e.g. skin, blood vessels, nerves, tendons, bone, etc.) and long periods of immobilization can inevitably cause stiffness and functional deficits. Firstly, a review of published literature on the topic was done. In particular, attention was given to prevention and physiopathology of stiffness, to the effects of tension applied in function to treatment intensity and duration, without forgetting basic rehabilitative procedures. Having identified the treatment with splints (low intensity for a long period of time) as the best procedure for this pathology, different types of tutors and splints were considered (static, dynamic and static-progressive), as well as the criteria to follow in selecting the procedure for their realisation. The experimental study was run at a physiotherapy centre dedicated to hand rehabilitation. I personally followed a group of twenty homogeneous patients affected by complex hand injuries with residual stiffness. Ten of these patients were treated with basic procedures, dynamic and static-progressive tutors. This group made up the study group. The other ten patients were treated with basic procedures and kinesiology therapy and formed the control group. The results were evaluated through the DASH questionnaire which was compiled at pre-treatment and post-treatment stages. The study group recorded significant improvement in the DASH values when compared to the control group: 40,6 versus 30,1. Such results have been presented with graphs and examined through a Student ?t? test which has confirmed the statistical significance (P = 0,0048). In conclusion from the experimental study carried out and from the literature reviewed the use of static-progressive and dynamic splints in addition to basic rehabilitative procedures have been identified as the best form of treatment for complex hand injuries. Specific preparation on the realisation and application of these devices is needful in training the modern physiotherapist who has decided to specialize in the rehabilitation of hand injuries.

TITOLO DEL LAVORO: LE ORTESI DINAMICHE E STATICO-PROGRESSIVE NEI TRAUMI COMPLESSI DELLA MANO. (Tipo di ricerca: sperimentale) Obiettivo della tesi č stato lo studio delle metodiche riabilitative ideali in caso di esiti di traumi complessi della mano: in tali evenienze il danno pluritessutale (cute, vasi, nervi, tendini, ossa) e i lunghi periodi di immobilizzazione causano inevitabilmente rigiditŕ e deficit funzionale. Si č proceduto innanzi tutto alla revisione della letteratura sull?argomento, in particolare relativamente alla prevenzione e fisiopatologia della rigiditŕ, quindi agli effetti delle tensioni applicate in funzione della intensitŕ e durata del trattamento. Sono state infine ricordate le procedure riabilitative di base. Avendo individuato nel trattamento con ortesi (basse intensitŕ per lunghi tempi) le procedure ideali per questa patologia, sono state descritti i tipi di tutori (statici, statico progressivi, dinamici), quindi i criteri per la scelta e le procedure per la loro realizzazione. Lo studio sperimentale si č svolto presso un centro fisioterapico dedicato alla riabilitazione della mano; in quella sede ho seguito personalmente un campione omogeneo di 20 pazienti affetti da esiti di traumi complessi della mano con rigiditŕ residua: 10 sono stati trattati con procedure di base e ortesi statico-progressive e dinamiche (gruppo di studio); altri 10 con procedure di base e chinesiterapia (gruppo di controllo). I risultati sono stati valutati con questionario DASH pre e post-trattamento: i valori di miglioramento del punteggio DASH sono risultati nettamente superiori nel gruppo di studio: 40,6 contro 30,1 del gruppo di controllo. Tali risultati sono stati visualizzati e presentati graficamente, quindi sottoposti a test ?t? di Student che ne ha confermato la significativitŕ statistica (P = 0,0048). In conclusione lo studio sperimentale eseguito, anche nel confronto con le evidenze di letteratura, ha individuato nelle ortesi statico-progressive e dinamiche lo strumento ideale, in aggiunta alle procedure riabilitative di base, per il trattamento degli esiti di traumi complessi della mano. Una preparazione specifica sulla realizzazione e applicazione di questi presidi risulta quindi indispensabile nella formazione di un moderno fisioterapista che decida di dedicarsi alla riabilitazione della mano traumatizzata.
