New trends in diabetic foot ulcers treatment

Abstract:

Úvod: Syndrom diabetické nohy (SDN) je jednou ze závažných pozdních komplikací diabetu mellitu. Proces debridementu je součástí komplexní péče o diabetickou ulceraci. Disertační práce je zaměřena na zhodnocení efektivity léčby pacientů se SDN ve vztahu k formě provedeného debridementu ulcerace. Metodika: Experimentální část výzkumu objektivizuje míru poškození měkkých tkání na spodině modelu rány při použití čtyř technik ostrého debridementu a porovnává stupně traumatizace testovaných vzorků tkání (n=128). Klinický výzkum byl zaměřen na populaci diabetiků (n=91) s ulcerací v oblasti dolní končetiny. Jeho cílem bylo porovnat klinickou účinnost debridementu ve skupinách ulcerací ošetřených skalpelem, hydrochirurgickým a autolytickým debridementem. Výsledky: Z analýzy dat experimentu vyplynulo, že debridement modelové ulcerace provedený pomocí hydrochirurgie dosahuje nižší úrovně traumatizace tkáně než nůžky a elektrokauter (p=0.00013, p=0.00145). Výsledky klinického sledování probandů potvrdily statisticky významný rozdíl v čase a počtu převazů potřebných ke kompletnímu vyčistění spodiny ulcerací - nejlepších výsledků dosáhla hydrochirurgická metoda debridementu (p <0.001). Hydrochirurgie také vedla ke statisticky významnému zkrácení celkové délky terapie a počtu převazů v porovnání s autolytickým debridementem (p=0.005). Závěr: Výzkum prokázal bezpečnost a efektivitu hydrochirurgické metody debridementu ulcerací syndromu diabetické nohy. Zkrácením celkové délky terapie se hydrochirurgie podílí na zvýšení kvality života pacientů s ulcerací SDN.

Abstract:

Background: Diabetic foot syndrome is one of serious late complications of diabetes mellitus. Debridement is the part of a complex diabetic ulceration treatment. The dissertation is focused on the evaluation of diabetic foot ulcer treatment effectivity against the debridement form. Methods: The experimental part of research is objectifying the soft tissue trauma degree on experimental wound model during sharp debridement procedure and comparing the tissue sample trauma degrees (n=128). The clinical part of our research concentrates on diabetic foot syndrome patient population (n=91). It’s aim is to compare a clinical effectiveness in sharp (scalpel), hydrosurgical and autolytic debridement groups. Results: The analysis of the obtained experimental data gave appealing information hydrosurgery method has reconcilable lower levels of wound surface damage and clinical efficiency related to wound healing than scissors and electrocauthery (p=0.00013, p=0.00145). We also found statistically significant difference in time and number of dressing changes needed for complete wound cleaning up – hydrosurgery had the best results (p ˂0.001). In comparsion with autolytic debridement the hydrosurgery has also reduced the total length and number of dressing changes (p=0.005). Conclusions: Research proved the safety and effectiveness of the hydrosurgical debridement in healing of diabetic foot ulcers. Reducing the total treatment length hydrosurgery increases the DFU patients quality of life.
