The use of mHealth applications by doctors and patients with type 1 diabetes in Czech Republic

Abstract:

Tato bakalářská práce se zabývá analýzou současného využití mHealth lékaři a pacienty s diabetem mellitem 1. typu v ČR a následnou možnou implementací mHealth do standardních procesů. V první části práce se definuje eHealth jako pojem pokrývající samotné mHealth. Dále se práce věnuje samotným mHealth aplikacím a zařízením. V další části je představeno onemocnění diabetes mellitus, rozdíly mezi 1. a 2. typem, jejich charakteristika a komplikace s tímto onemocněním spojené. Následující kapitola se věnuje historii a vývoji terapie DM 1. typu, stejně jako vývoji konkrétních zdravotních pomůcek, které tvoří základní kameny léčby v dnešní době. Čtvrtá kapitola bakalářské práce popisuje současnou situaci z pohledu úhrady zdravotních pomůcek pro pacienty s DM 1. typu v ČR zdravotními pojišťovnami. V další části se bakalářská práce věnuje samotné analýze využití mHealth pacienty a lékaři. A to jak na základě dat získaných prostřednictvím dotazníkového šetření na téma mHealth, tak formou rozhovoru s diabetology. Následně je také detailně analyzován současný standardní postup péče o pacienty, s popisem průběhu standardních kontrol a jejich součástí. V poslední kapitole se práce blíže zabývá možnou implementací mHealth do současných procesů popsaných v předchozí kapitole. Současně se v této kapitole nachází návrh a opodstatnění rozšíření současných mHealth aplikací pro jejich případné zatraktivnění. Tato část poskytuje náhled na jejich hlavní výhody a limitace, z nichž dále vyplývají konkrétní navržená rozšíření. Ta berou v potaz a co nejvěrněji odrážejí zjištěné údaje od lékařů a pacietů s diabetem 1. typu.

Abstract:

This bachelor thesis focuses on an analysis of the current use of mHealth by doctors and patients with type 1 diabetes in Czech Republic and a subsequent potential implementation of mHealth into standard processes. The first part of the thesis defines eHealth as a term covering mHealth itself. Then the thesis focuses on particular mHealth applications and devices. The next section presents diabetes mellitus, the main differences between type 1 and 2, their characteristics and complications associated with this disease. The following chapter describes the history and evolution of type 1 DM therapy, as well as the evolution of specific medical devices that are part of the foundation of today's treatment. The fourth chapter of the bachelor thesis describes the current situation when in terms of reimbursement of medical devices for patients with type 1 DM in Czech Republic by health insurance companies. In the next part, the bachelor thesis focuses on the respective analysis of the use of mHealth by patients and doctors. And that is based on data acquired through an online questionnaire survey on mHealth and through an interview with practitioning diabetologists. Then, the current standard patient care process is described and analyzed as well with a description of the course of standard checks and their components. In the last chapter the thesis tries to find a possible implementation of mHealth into current processes described in the previous chapter. At the same time, this chapter contains a proposal and justification for the extension of current mHealth applications that could possibly enhance their attractiveness. This section provides an overview of their main advantages and limitations, from which the specific proposed extensions follow. These take into account and reflect as closely as possible the data obtained from physicians and patients with type 1 diabetes.
