Morphology of Cranial Gunshot Traumas: Exploring the Influence of External and Internal Factors - a DFM Brno case study (2022-2023)

Abstract:

Častým předmětem zkoumání soudního lékařství a forenzní antropologie jsou střelná poranění lebky. Pro správnou interpretaci těchto poranění je důležité pochopení jejich morfologie. Jedním ze způsobů hodnocení morfologických znaků poranění je popis určitých rozměrů kostí a souvisejících měkkých tkání v blízkosti poranění. Měření těchto rozměrů se může provádět mimo jiné na post-mortem CT (PMCT) záznamech. Tématem této práce bylo zhodnocení vlivu vnějších a vnitřních faktorů na morfologii střelného poranění lebky pomocí těchto záznamů. Cílem praktické části práce bylo změření a analýza morfologie střelných poranění lebky na PMCT záznamech a určení její možné závislosti na biologických faktorech zemřelého (tloušťka diploe, věk, tloušťka měkké tkáně) a vnějších faktorech (ráže). PMCT záznamy byly vytvořeny v rámci soudně-lékařského vyšetření na Ústavu soudního lékařství v Brně. V pro-gramu 3D Slicer byly na řezech a 3D modelech změřeny vybrané rozměry střelných otvorů a data byla následně vyhodnocena ve statistickém programu R. Analyzováno bylo celkem 21 případů, které byly v průběhu vyhodnocování rozdělovány na základě různých kritérií (poloha střelného otvoru, zda šlo o průstřel nebo o zástřel). Podle Spearmanova koeficientu pořadové korelace nebyla u žádné z dvojic proměnných zaznamenána statisticky významná korelace. Tento výsledek ale může být ovlivněn nízkým počtem analyzovaných případů. Tato práce přispívá k chápání morfologie střelných poranění lebky a potřeby zvážení biologických a vnějších faktorů v průběhu vyšetřování. Z výsledků vyplývá potřeba dalšího výzkumu těchto faktorů a jejich vlivu na vzhled střelných ran. Rozšíření této studie by mohlo být zaměřeno na analyzování většího množství případů nebo na rozšíření spektra hodnocených znaků

Abstract:

Gunshot wounds to the cranium are a frequent subject of investigation in forensic medicine and forensic anthropology. An understanding of the morphology of these injuries is important for their correct interpretation. One way of assessing the morphological features of the injury is by describing certain dimensions of the bones and associated soft tissues close to the injury. The measurement of these dimensions can be performed, among other things, on post-mortem CT (PMCT) recordings. The topic of this study was to evaluate the influence of external and internal factors on the morphology of gunshot wounds to the skull using these recordings. The aim of the practical part of this study was to measure and analyse gunshot wounds of the skull on PMCT scans and to determine their possible dependence on biological factors of the deceased (diploe thickness, age, soft tissue thickness) and external factors (caliber). The studied PMCT records were created during the forensic medical examination at the Institute of Forensic Medicine in Brno. Selected dimensions of the gunshot holes were measured on slides and 3D models in the 3D Slicer program and the data were subsequently evaluated in the statistical program R. A total of 21 cases were examined and categorized based on various criteria during the evaluation. According to Spearman's rank correlation coefficient, no statistically significant correlation was observed for any of the pairs of variables. This work contributes to the understanding of the morphology of gunshot wounds to the skull and of the need to consider biological and external factors during the investigation. The results suggest that further research into these factors and their influence on the appearance of gunshot wounds is needed. The followig research could be focused on examining higher number of cases, or on broadeing the spectrum of analyses morphological feature.
