Vyšetření chromozomálních poruch spermií a preimplantačních embryí metodou fluorescenční in situ hybridizace

Analysis of chromosome abnormalities in sperm and preimplantation embryos using the fluorescence in situ hybridization method

Abstract:

Metoda mnohobarevné interfázní fluorescenční in situ hybridizace (iFISH) umožňuje vyšetření chromosomů jader blastomer preimplantačních embryí a v kombinaci s dekondenzací jádra umožňuje rychle analyzovat různé chromosomální abnormality ve velkých vzorcích spermií. Pro zjištění interindividuální variability ve frekvenci aneuploidií chromosomů ve spermiích a diploidních spermií byla opakovaně vyšetřována skupina mladých zdravých normozoospermických mužů. Potvrdili jsme mezichromosomové odlišnosti ve frekvenci disomií a existenci stabilních variant – zdravých mužů s konstantně zvýšenou produkcí aneuploidních spermií. Zjištěné výsledky frekvence aneuploidií ve spermiích byly použity jako kontrolní při srovnání v dalších studiích. Ve spermiích tří mužů, nositelů dvou různých balancovaných reciprokých translokací chromosomů 11 a 18, byla analyzována meiotická segregace translokovaných chromosomů a frekvence aneuploidií některých dalších chromosomů. Potvrdili jsme, že každá reciproká translokace má specifické schéma segregace, ale příbuzní nositelé stejné reciproké translokace vykazují podobné schéma v příslušných segregačních frekvencích. V praxi je možné výsledky cytogenetického vyšetření spermií nositelů translokace využít v genetickém poradenství a reprodukční medicíně. Při preimplantační genetické diagnostice (PGD) byl u dvou ze čtyř embryí zjištěn polymorfní třetí menší signál alfa-satelitní sondy pro chromosom 18. Při podezření na polymorfismus při PGD analýze lze využít sond pro jinou oblast daného chromosomu k potvrzení diagnózy nebo vyloučení chybné diagnózy. Rozpoznání daného typu polymorfismu při PGD je nezbytné pro správnou klasifikaci vyšetřovaných embryí. U fenotypicky normálního nositele malého nadpočetného paternálně zděděného marker chromosomu 15 byl tento marker chromosom a další vybrané chromosomy vyšetřovány ve spermiích a embryích. Ze zjištěných výsledků vyplývá, že segregace marker chromosomu se může lišit od teoreticky očekávaného poměru 1:1, přítomnost marker chromosomu nemusí zvyšovat aneuploidie ostatních chromosomů a nemusí negativně působit na vývoj časných embryí. iFISH se ukázala jako hodnotný výzkumný nástroj pro stanovení frekvence aneuploidií, pro segregační studie u nositelů balancovaných translokací a marker chromosomů, i pro vyšetření preimplantačních embryí.

Abstract:

The multicolour interphase fluorescence in situ hybridization (iFISH) method is suitable for the investigation of chromosomes in embryonic blastomeres; and in combination with nuclei decondensation, for rapid detection of chromosomal aberrations in a large number of sperm. To examine interindividual differences in sperm chromosome aneuploidy and diploidy, repeated semen specimens were obtained from a group of young, healthy and normozoospermic men. Our findings confirmed inter-chromosome differences in the frequencies of disomy and suggested that some apparently healthy men exhibit consistently elevated frequencies of specific sperm aneuploidies (stable variants). This group of men was also used as a control group in other analyses of aneuploidies in sperm. In the sperm of three men who were carriers of two different translocations involving chromosomes 11 and 18, we analysed meiotic segregation of the translocated chromosomes and frequencies of aneuploidy of other chromosomes. We confirmed that each type of reciprocal translocation has a specific pattern of segregation, but related carriers of the same translocation showed a similar pattern in the respective segregation frequencies. Under practical conditions, the obtained results can be used in genetic counselling and reproductive medicine. A polymorphic third small signal of chromosome 18 was detected in two of four tested embryos by preimplantation genetic diagnosis (PGD). In cases of suspected polymorphism in PGD analysis, the use of probes for another region of the particular chromosome is recommended to verify the results and to prevent misdiagnosis. The recognising of the given type of polymorphism in PGD is necessary for the right classification of examined embryos. In a phenotypically normal male carrier of a small, paternally inherited supernumerary marker chromosome 15, we examined this marker chromosome and other selected chromosomes in sperm and embryos. It follows from our results that segregation of the marker chromosome can be different from the expected ratio 1:1; it does not increase aneuploidy of other chromosomes, and does not adversely affect embryo development. The iFISH method showed itself to be a valuable tool in the assessment of aneuploidy, for segregation studies in balanced translocation carriers, in carriers of marker chromosomes, and also for the examination of preimplantation embryos.
