Effect of treatment with low molecular weight heparin on the laboratory diagnosis of Lupus anticoagulant

Abstract:

Antifosfolipidový syndrom je autoimunitní onemocnění, způsobené tvorbou protilátek proti vlastním fosfolipidům. Toto onemocnění patří mezi nejčastější trombofilní stavy a je charakterizované vyšší tendencí vzniku trombóz. Mezi antifosfolipidové protilátky patří Lupus antikoagulans, protilátky proti kardiolipinům a protilátky proti ?2-glykoproteinu I. Diagnostika Lupus antikoagulans není jednoduchá, protože neexistuje test, který je 100% citlivý jen na Lupus antikoagulans. K prodloužení testů, které jsou závislé na fosfolipidech dochází i u jiných trombofilních stavů a také vlivem léčby. Bakalářská práce se dělí na teoretickou a praktickou část. Teoretická část pojednává o hemostáze, trombofilních stavech, nízkomolekulárních heparinech a také o antifosfolipidovém syndromu. Praktická část bakalářské práce se zaměřuje na laboratorní diagnostiku Lupus antikoagulans pomocí screeningových, směsných a konfirmačních testů na analyzátoru Sysmex CN-6000. Analýza byla provedena na Oddělení klinické hematologie Ústavu laboratorní medicíny ve Fakultní nemocnici Ostrava. Výsledky byly graficky znázorněny a statisticky byla vyhodnocena specificita testů. Procentuálně bylo vyhodnoceno zastoupení falešně pozitivních výsledků k celkovému počtu pacientů. Specificita laboratorní diagnostiky Lupus antikoagulans byla vyhodnocena jako 86,1%. Celý soubor dat zahrnuje 57 vzorků pacientů léčených nízkomolekulárními hepariny. Stanovením cut off 0,299 IU/mL pro aktivitu anti-Xa nízkomolekulárního heparinu bylo vybráno 37 vzorků. Výsledky Lupus antikoagulans byly interpretovány v souladu s kritérii pro vyhodnocení a bylo stanoveno 5 falešně pozitivních výsledků, tj. 13,5 %. Z výsledků vyplývá, že laboratorní diagnostika Lupus antikoagulans testy prováděnými ve Fakultní nemocnici Ostrava může být ovlivněna léčbou nízkomolekulárními hepariny.

Abstract:

Antiphospholipid syndrome is an autoimmune disease caused by the formation of antibodies against the body's own phospholipids. This disease is one of the most common thrombophilic conditions and is characterized by a higher tendency to thrombosis. Antiphospholipid antibodies include Lupus anticoagulant, anti-cardiolipin antibodies, and anti-?2-glycoprotein I antibodies. Determining Lupus anticoagulant is not easy, because there is no test that is 100% sensitive only for Lupus anticoagulant. Prolongation of tests that are dependent on phospholipids also occurs in other thrombophilic conditions and treatments. The bachelor thesis is divided into theoretical and practical parts. The theoretical part deals with haemostasis, thrombophilic conditions, low molecular weight heparins and also antiphospholipid syndrome. The practical part of the bachelor thesis is focused on the laboratory diagnosis of Lupus anticoagulans using screening, mixed and confirmatory tests on the Sysmex CN-6000 analyzer. The analysis was performed at the Clinical Hematology Department of the Institute of Laboratory Medicine at the University Hospital Ostrava. The results were graphically plotted and the specificity of the tests was statistically evaluated. The percentage of false positive results to the total number of patients was evaluated. The specificity of laboratory diagnosis of Lupus anticoagulans was evaluated as 86,1%. The entire data set includes 57 samples of patients treated with low molecular weight heparins. By determining a cut off of 0,299 IU/mL for the anti-Xa activity of low molecular weight heparin, 37 samples were selected. Lupus anticoagulant results were interpreted according to the evaluation criteria and 5 false positive results were determined, i.e. 13,5%. The results suggest that the laboratory diagnosis of Lupus anticoagulans tests performed at the University Hospital Ostrava may be influenced by treatment with low molecular weight heparins.
