Binocular vision correction, measurement and correction of heterophoria using MKH method

Abstract:

Pro hodnocení stavu kompenzace heteroforií vzniklo několik metodických přístupů a analytických systémů. Jedním z nich je Měřicí a korekční metodika podle H.-J. Haaseho (MKH), která umožňuje měřit motoricky kompenzovanou i senzorickou složku heteroforie a stanovit podtyp fixační disparity dle teorie MKH. Hlavním cílem této studie bylo posoudit závislost fúzních vergencí na velikosti motorické složky heteroforie do dálky, která byla vyjádřena asociační heteroforií měřenou na Křížovém testu. Dalšími cíli bylo posouzení závislosti Asociační heteroforie měřené metodou MKH a asociační heteroforie měřené na Křížovém testu na věku a refrakční vadě, stanovení výskytu asociační heteroforie a stanovení procentuálního zastoupení podtypů fixační disparity podle MKH. Tato průřezová studie zkoumala soubor 167 subjektů o širokém věkovém rozpětí (12–78 let) s průměrným věkem 40.6±16.75 let. V této studii byl nalezen vysoký výskyt asociační heteroforie podle MKH (85 %), která téměř v jedné třetině obsahovala vertikální složku odchylky. Celkový rozsah fúzních vergencí do dálky se lišil mezi exoforiemi, ortoforiemi a esoforiemi. Nejvyšší hodnoty pozitivní i negativní fúzní vergence byly nalezeny u esoforie, nižší u ortoforie a nejnižší u exoforie. Pro klinickou praxi z toho vyplývá, že by při stanovení kompenzace heteroforií využitím porovnání nálezu s normativními hodnotami nebo kritérií, která srovnávají velikost heteroforie s opozitní fúzní vergencí, měl být zohledněný směrový typ a velikost heteroforie. Výsledky studie ukázaly, že u exoforie nebyly zvětšeny pozitivní fúzní vergence kompenzující heteroforii v závislosti na její velikosti a u esoforie byly negativní fúzní vergence zvýšeny pouze nevýznamně. Z toho lze usuzovat, že heteroforie do dálky není doprovázena zvětšením schopnosti její kompenzace pomocí fúzních vergencí. Proto je u asymptomatického pacienta, po zvážení vlivu korekce případného refrakčního deficitu, vhodná další intervence v podobě prizmatické korekce, sférického přídavku do blízka nebo zrakových tréninků. Nebyl nalezen vztah mezi velikostí heteroforie a věkem nebo refrakční vadou. Z toho vyplývá, že klinická pozornost by měla být věnována pacientům bez ohledu na jejich věk nebo typ a velikost refrakční vady.

Abstract:

There are several methodological approaches and analytical systems that have been developed for assessing the condition of compensation of heterophorias. One of them is Measuring and Correcting Methodology after H.-J. Haase (MKH), which facilitates measuring motor-compensated as well as sensor components of heterophoria, and determines subtypes of fixation disparity according to the theory of MKH. The main aim of this study was to assess dependence of fusional vergences on the amount of the motor component of distance HTF, which was expressed by associated HTF as measured with the Cross test. Additional aims involved assessment of dependence of associated heterophoria of MKH method and associated heterophoria measured using the Cross test on the age and refractive error, then determination of associated heterophoria, and determination of percentage representation of fixation disparity due to the theory of MKH subtypes. This cross-sectional study examined a group of 167 subjects of a wide age range (12–78) with the average age of 40.6±16.75. The study has found very frequent occurrence of associated heterophoria measured by MKH method (85 %), in almost one-third of all there was a vertical component of the deviation. The amount of distance fusional vergences was different between the groups of exophoria, orthophoria and esophoria. The highest values of the positive and negative fusional vergence were in esoforia, lower values in ortoforia and the lowest values in the exoforia group. In clinical praxis in estimating the compensation of the heterophoria there is importantant not only to compare the results with using standard tables of expected values of opposite fusional vergence, but to take into the account the direction of horizontal deviation and the amount of horizontal heterophoria. The study has not proved increase in positive fusional vergence in exophoria group due to compensation. In the esophoria group was the value of the negative fusional vergence elevated statisticaly not significant. Such results indicate, that there is no compensating elevation of the fusional vergence in distance heterophoria. In addition, the study has not proved any dependence of the distance associated heterophoria amount on refractive error on age. According to these findings, associated heterophoria does not occur only with high refractive errors and in elderly patients. This supports the appropriateness of performing screening with every client, primarily in case of typical symptoms of binocular function disorders.
